Some Seniors Are Reluctant to Explore Assisted Living

Way too often older adults have anxiety about assisted living that is based on misinformation. While we acknowledge assisted living is a big change for a senior as it may mean leaving behind a beloved family home, it doesn’t mean an end to their privacy and independence. Interacting With Individuals Who Have Alzheimer’s Disease

A Yale Medicine neurologist offers advice on how to better connect with someone with Alzheimer’s disease. Carolyn Fredericks, MD., a specialist in cognitive and behavioral conditions, including Alzheimer’s, says it is not always helpful to correct a person with Alzheimer’s, and in most cases, it is nicer to just “enter their reality.” 

Fredericks suggests instead of correcting the person when they get your name or someone else’s name incorrect, just go with it. After all, what difference does it really make? Also, you don’t need to remind the person you answered their question an hour ago. Let it go and repeat the answer.  

Going along with what the person says, and remaining calm and encouraging will help them remain calm. Research indicates someone with Alzheimer’s is typically most anxious about making mistakes or forgetting something they should know in the early stages of the disease. Moreover, when they repeat a question, they are not trying to be difficult, so there is no reason to make them feel uncomfortable. If the person is unduly worried about something that is not true such as thinking things that go “missing” are being stolen, show compassion and focus on helping them find the items.   

As long-term memories are more intact in people with Alzheimer’s and often not affected until the later stages of the disease, spending time reminiscing about things from their childhood can be fun for you and the person. Even in advanced stages of Alzheimer’s, people find joy in listening to their favorite music or participating in creative projects. Be sure they get some exercise each day as it helps with anxiety and sleep. Lastly, Dr. Frederick says it is important for the family to construct a safety plan and recognize when it is no longer safe for the person to drive or use the stove when he/she is alone.

Exercising for Even a Few Minutes Each Day Can Have Positive Effects

Studies continue to show that getting enough sleep, following a healthy diet, quitting smoking, managing stress, challenging ourselves mentally and staying physically fit are beneficial for our overall health. Now a study finds it doesn’t take a large amount of physical exercise each day to protect our brain health as we age. The article was published in the journal, Age and Ageing, which showed that as little as 5 minutes of moderate-to-vigorous physical activity daily can help keep the aging brain healthy.

Experts explain as we grow older, the outer layer of our brain known as the cortex becomes thinner and the brain typically starts producing lower levels of neurotransmitters, such as dopamine, which can contribute to cognitive decline. Audrey M. Collins, PhD, a postdoctoral researcher in the Department of Neuroscience at the AdventHealth Research Institute in Florida, explains, “Physical activity is a lifestyle behavior that may play a role in not only the prevention and attenuation of cognitive decline, but the promotion of cognitive function in late life.” 

The study found participants who did moderate-to-vigorous physical activity daily had significantly better brain processing speed, short-term memory and better executive function, which is the ability to plan and focus. However, Collins says the study also discovered the biggest cognitive gain came from those participants who changed their daily behavior from not doing any moderate to vigorous physical exercise to exercising at least five minutes a day.  

Gary Small, MD, chair of psychiatry for Hackensack University Medical Center in New Jersey and author of more than a dozen books on brain health and aging, commented on the study by saying it is important for people to exert themselves enough to push the heart and lungs to pump oxygen and nutrient to the brain. It should also be reassuring to people that they do not have to become triathletes to maintain good health.

Rather than trying to look 20 years younger, more seniors are embracing the aging process with a priority on healthy living as they begin the journey into their golden years as gracefully as possible. There is an understanding that it is not just about how long we live but a move toward ensuring our later years are vibrant, fulfilling and purposeful.  

Experts explain our bodies naturally go through changes as we age but there are things we can do to navigate those changes to help maintain health and enhance quality of life. One way to do this is to adopt the attitude that old age is a privilege. Rather than wasting time complaining about all you are dealing with, remember to look at your senior years as an opportunity to grow wiser, correct past mistakes and move with purpose. Also, if you practice gratitude, it will help you remember it isn’t that bad growing older and sure beats the alternative.

Moreover, what better time is there than now to do those things you have been talking about doing for years. You can learn to play an instrument, you can take dance lessons, you can volunteer for a cause that is close to your heart; or whatever else you want to do. Studies show seniors who are learning new things and are staying curious and engaged report they are happier and more satisfied with their lives. 

Other ways to age more gracefully and healthfully are to watch your gut health by eating more vegetables, fruits, lean proteins and fiber. Try to get at least 150 minutes of moderate exercise each week as it can help maintain bone density, flexibility, balance and strength. Exercises such as Tai chi, Pilates and yoga can help strengthen your core and improve your posture and balance as well as lessen backaches. Lastly, get recommended health screenings and vaccines.

Nutritious Options for Snacks

Many of us experience a mid-morning or late afternoon slump in terms of energy and need a little bite of something to give us a lift. While experts say snacking can be a good thing, it is important to do it with healthy options. They emphasize the best snacks are low-sugar and nutrient-rich; and provide energy as well as support overall health. Snacks rich in fiber and protein help keep us feeling full. Foods to avoid are ultra-processed and high in calories, fat, sodium and added sugar. 

Some examples of healthy snacks are smoothies. They can be made with fresh greens, your choice of milk and fruit. Along the same line is a cup of low-fat yogurt topped with fresh or frozen, thawed berries. If you don’t like yogurt, a cup of cottage cheese with fruit is a good alternative. 

Kale is one of the healthiest leafy, green vegetables; and kale chips are a fun way to enjoy it. Moreover, they are very easy to make on your own. Prep carrots, celery and other raw veggies in advance to dip into humus or low-fat yogurt dip. Other quick and easy snacks include steamed edamame, unsalted pepitas, nuts and fresh fruits. 

The popular superfood, chia seeds, are loaded with protein and fiber, and support heart and brain health. One tasty way to eat them is as a chia pudding, which is another very simple thing to make. Other quick snack ideas include peanut butter and apple slices or guacamole spread on whole wheat toast. 

Whatever snack you decide on, make it something you enjoy. Registered dietitian at the Cleveland Clinic Department of Wellness & Preventive Medicine, Kristin Kirkpatrick, M.S., R.D., L.D., says regardless of how good it is for you, if you don’t like it, you probably won’t eat it. She explains that “studies show that when you take this important factor into your dietary pattern, you are more likely to stick with it. For example, if you don’t like kale, don’t include it—instead, find another green or cruciferous veggie that you love.”

As We Age, Exercise Helps Maintain Mobility

Normal aging brings about a decline in our physical abilities that affect strength, stamina and speed – all associated with declining muscle mass. Harvard Health Publishing explains that there is not much decline in our muscles between the ages of 20 and 40; after age 40 there can be a decline of up to 1% to 2% per year in lean body mass and 1.5% to 5% per year in strength. 

Mobility is key to protecting our independence. Along with losing muscle mass, other common problems in older adults include poor reflexes and balance, and loss of flexibility. Fortunately there are things we can do to encourage a healthier, safer aging process. 

One of the first things is to avoid a sedentary lifestyle. One prevailing myth many older adults believe is physical activity will lead to injury or a fall. Experts say the actual truth is too much sitting causes muscle loss and decreased flexibility. For older adults, it is important to exercise regularly to compensate for bodily changes in hormones and other factors that occur with aging. 

Strength exercises using light weights or bands can help protect bones, build muscle and improve flexibility and balance along with aerobics, stretching and special balance exercises.  These include things such as brisk walking, jogging, biking, swimming, or cardio classes. To help build strength, balance, and flexibility, consider a class in yoga, tai chi, Pilates or isometric weightlifting. 

In addition, regular exercise not only helps maintain our mobility but is associated with a stronger heart and less occurrence of cancer. Talk to your healthcare provider or a physical therapist regarding the best exercises for your needs. The CDC recommends adults get at least 150 minutes of moderate-intensity physical activity per week.

Vascular Dementia

While most of us are familiar with Alzheimer’s disease, there are other less common forms of dementia with one of those being vascular dementia. Vascular dementia is a general term describing problems with reasoning, planning, judgment, memory and other thought processes. It is often caused by brain damage from impaired blood flow to the brain like during a stroke, but thankfully it depends on the location and severity of the stroke damage. Other conditions that can harm blood vessels, reduce circulation, and deprive the brain of oxygen and nutrients, are a brain hemorrhage caused by things like high blood pressure as well as abnormal aging of blood vessels (atherosclerosis) and diabetes. 

The Mayo Clinic explains vascular dementia can imitate many of the symptoms of Alzheimer’s disease but the biggest difference of vascular dementia is people suffer a greater loss in the speed of organizing thoughts and problem-solving rather than actual memory loss. The condition can come on suddenly such as after an event like a stroke or as a more gradual decline like Alzheimer’s. Researchers also have found many people with telltale signs of brain vascular disease also have Alzheimer’s disease.  

Symptoms of vascular disease include things like: 

  • Confusion

  • Trouble paying attention and concentrating

  • Reduced ability to organize thoughts or actions

  • Decline in ability to analyze a situation, develop an effective plan and communicate that plan to others

  • Slowed thinking

  • Difficulty with organization

  • Difficulty deciding what to do next

  • Problems with memory

  • Restlessness, agitation and depression

  • Unsteady gait 

The condition is rare in those under 65 and increases significantly in those over 90 years of age. To mitigate your risk, doctors say maintain a healthy blood pressure, keep weight and cholesterol in check and prevent or manage diabetes. If you smoke, quit; and engage in regular physical activity. A healthy low-fat diet also is a factor in preventing heart disease and strokes, which in turn helps prevent vascular dementia.

Tips to Detect and Protect Yourself from Cybercrimes

Unfortunately, cybercrimes are on the rise. From 2020 to 2022, statistics show there was a 25 percent increase in reports of phishing. Of those, 42 percent were made either through an email or text.  

Fortunately, there are tips to help you identify phishing messages. One of the first things to look for is a text or email that uses a generic greeting such as an “hello (a name of an organization or company) member without identifying you by name.   Another frequently used tactic is to add a sense of urgency that action needs to be taken NOW in order to prevent a breach in your security. Also remember any attachment may contain malware so the safest thing is to not open it unless you are absolutely confident in the sender.

Before assuming an email or text is legitimate, experts say to look for the company’s logo on the document and never open an URL that looks scrambled. Be suspicious of poorly written messages with misspellings or improper grammar as well as asking for personal information. Never give out account or credit card numbers, your social security number and other important financial information over a text, email or the phone. 

Your approach to all of your account info, passwords, or validation codes are for your eyes and ears only and even a legitimate customer service rep is not entitled to them. Don’t return automated calls or count on caller ID being legit as they too can be fake. As cybercrimes have evolved, criminals have gotten more sophisticated creating fake https and the “s” no longer ensures a safe site. Also, we are all used to counting on seeing the padlock symbol on the site, but that too is being fraudulently created. Experts advise typing in the URL yourself to ensure you reach a reputable site. If you think you are a victim of a cybercrime, don’t click on any links, call any listed phone numbers, or download attachments; and report it to the company it claims to be from.

Loneliness can be Dangerous to Our Mental & Physical Health

A recent study published in the Journal of the American Geriatric Society, explains loneliness may be more detrimental to our overall health than smoking, obesity and alcoholism, or leading a sedentary lifestyle. The study was conducted by the Regenstrief Institute data informatics firm and the Indiana University School of Medicine. What is even more concerning is their research found a majority of those surveyed (53 percent) considered themselves lonely.  

The researchers call loneliness a “biophysical stressor,” and say they hope more attention to this problem will motivate doctors to address this often overlooked lifestyle factor that negatively impacts seniors’ quality of life. One of the study authors, IU research scientist and professor Monica Williams-Farrelly, said, “So in the same way that we ask older adults: ‘Do you smoke? Or do you measure your blood sugar?’ We should be inquiring about and measuring loneliness and offering solutions.”

Researchers say participants who identified as being lonely experienced lower mental and physical health outcomes across both demographics and health conditions. Moreover, they say while there may be a connection between those who report feeling lonely to poor eating habits, overindulging in alcohol or not exercising; there still exists a strong database showing loneliness strongly influences our health and well-being. In fact, the recommendation from the study’s authors is to stress the fact that loneliness should be treated as a serious health factor by our communities as well as by medical professionals.

Starting at age 65, Harvard Publishing says there is a 70 percent chance a person will need long-term care at some point during their remaining years. However, it is important to take into account they have limitations in terms of loudness and features; and are probably not appropriate for those with severe hearing loss or for complex listening situations. 

It is estimated 48 million Americans have some form of hearing loss with more than 90% of these cases falling into the category of mild to moderate hearing loss. Due to the high costs of hearing aids and the fact the majority of cases are mild to moderate, the U.S. Food and Drug Administration (FDA) established a new category of over-the-counter (OTC) hearing aids in 2022 as a way to more affordably serve those with mild to moderate hearing loss. 

A recent article in Forbes magazine takes on the topic of OTC hearing aids, listing some of the best and things you should know. For example, they say you should know OTC hearing aids are not the same as personal sound amplification products (PSAPs), which are used to amplify sounds in certain environments and are not subject to FDA regulation. Also to see if you would be a good candidate for an OTC hearing aid, Michele Michaels, a hearing health care program manager at the Arizona Commission for the Deaf and the Hard of Hearing in Phoenix, recommends consulting with a hearing health professional or your primary care physician. Your hearing loss could be as simple as a build-up of earwax or from other causes, which could be detected through an ear exam and/or a hearing screening.   

Michaels talks about common hearing problems that could be addressed with an OTC hearing aid such as not hearing the first letter of a word. For instance, hearing the word tool or school in the sentence, “Let’s go to the pool.” Other signs of a hearing loss are if you have to ask people to repeat themselves or have trouble understanding when someone whispers to you. You may routinely confuse something like 15 minutes from 50 minutes in a conversation or need to sit closer to hear properly. To get started with better hearing, talk to your healthcare provider.

The Advantages of Stretching Exercises

If you are a senior who is having difficulty turning your neck to look behind you when driving, having problems getting up from a low chair or having difficulty picking something up from the floor, you may find adding stretches to your routine can really improve these types of motions. Stretching exercises also help seniors improve flexibility and balance, both of which lower a person’s risk of falls and injuries. Experts explain stretching gives seniors a better range of movement in their joints, helps improve posture, increases circulation and releases muscle tension and soreness. 

The Journal of Gerontology reported on the results of a 12-month stretch and flex program for older adults. It found “participants demonstrated positive changes in areas like physical fitness, self-efficacy, perceived functioning, and well-being. They also experienced a decrease in pain.” Along with the favorable benefits stretching provides, it can be done anywhere and does not require any special equipment. However, before beginning any new exercise routine, be sure to consult with your healthcare provider regarding your fitness level and the type of stretches most appropriate for you. 

Another thing to remember is it is important to warm up before stretching. Experts say it can be doing something as simple as a walk around the block. Additionally, they warn us not to stretch to the point of pain, only to the point of tension. A stretch can typically be held for 30 seconds, but do not bounce when you stretch.

The Harvard Health Publishing journal reported on a study focused on the effectiveness of music therapy. The study found listening to music, singing and other forms of music therapy “can create significant improvements in mental health and smaller improvements in physical health–related quality of life.” Researchers sum it up by saying music offers a positive impact on the psychological quality of life; and is an easy way to raise spirits and help us feel better.   

So if we feel music makes us happier and more relaxed, it is not our imagination. AARP quotes Laurie Keough, a clinical associate professor of music therapy at Nazareth College in Rochester, New York, who explains, “Music is processed all throughout the brain;” which is why it is applicable in so many different situations. 

Music therapy is an established form of therapy with a lot of evidence of the benefits. These include improving health care goals like decreasing anxiety, improving mood and decreasing pain perception among others. Since listening to music is one of the best ways to get the benefits; it is easy for all of us to do.

Researchers say as we listen to music, it triggers the release of dopamine, a neurotransmitter that’s part of the brain’s reward system, making us feel happier. It can also distract from negative thoughts and help us focus on more pleasurable emotions. A soothing playlist can be used to quiet the mind to help transition to sleep. Moreover, as music is known to bridge social connections, many senior living communities hold group sing-a-longs, which are not only fun but support studies showing singing improves language, memory and processing of information.  

Other studies show the power of music to trigger memories in people living with dementia. It can help them feel connected to others, even when verbal communication is difficult as music memories remain in the brain long after other memories fade. An article in the US News & World Report finds moving mom or dad to an assisted living community may be a bargain compared to paying for extensive in-home care. With an average cost of $25 to $27 an hour, in-home care can add up fast. For example, if you want someone with mom or dad while you are at work nine hours a day for five days a week, you are already spending $4,500 or more a month. This still leaves your parents without support on the weekends, and doesn’t include any support for problems which may occur during the evening and overnight. 

Assisted living communities include room and board and housekeeping services as well as a variety of wellness programs and activities. They also have 24/7 staffing and suites with emergency call systems, handicapped-accessible bathrooms and other modifications.  

If a family feels assisted living would be the best place for a parent but the cost is holding them back, experts say to thoroughly research all available payment options. This can include long-term care insurance and some private health insurance; or social programs such those offered by Veterans Affairs or Social Security Disability Insurance. A San Francisco based executive VP for A Place for Mom, Sue Johansen, suggests families consult with a financial advisor or a trust attorney. 

Remember, once you find the assisted living community you want, ask about the difference in pricing regarding room size or floor plan, and/or location in the community (view, balcony, etc.). You will need to factor in annual increases so be sure to ask what those have been over the last couple of years. What is Health Span?

Studies show more and more people are living over a decade or more in poor physical and mental health, often making it difficult to care for themselves. Since we are living much longer than our ancestors, scientists are looking at ways to help us live a healthier, more active life. An article in National Geographic writes about current research focused on trying to boost a person’s active, disease-free years. The concept, known as “health span,” has researchers looking at ways to “understand the underlying mechanisms of aging, working to identify health-promoting genes, and zeroing in on steps in our daily lives to improve our health span.”  

While over the last few years we have seen articles promoting drugs as a way to increase longevity, Sharon Inouye, a Harvard Medical School physician-scientist who looks at aging issues, says prevention may be the clue to a longer health span. Another scientist, Nir Barzilai, associated with the Albert Einstein College of Medicine in the Bronx, New York, says, rather than promoting the use of longevity drugs; we need to prioritize exercise, nutrition, sleep and social connectivity to enhance overall health.   

If prevention is the answer, then health experts say it would be beneficial for people to start being monitored in their 30s or 40s for baseline health data such as vitamins, hormones, and blood sugar levels. Knowing our future health risks could help us make better decisions about how we live our lives now.

Dizziness & Older Adults

Doctors say dizziness is a common complaint of older adults. Symptoms may include vertigo (feels like the room is spinning); light-headedness; faintness and weakness; and nausea. While confusing and uncomfortable to sufferers, experts say it’s rarely a sign of anything serious or life-threatening, and is generally treatable. However, if you experience acute, prolonged dizziness, especially if accompanied by symptoms such as chest pains, passing out and/or severe headache; seek immediate medical attention. 

Most causes of dizziness are from an inner ear disturbance, motion sickness, medications, or other conditions of poor health. To diagnose the problem, your doctor will want to know how long your dizziness lasts and other symptoms you may be having. Medications with the known side-effect of dizziness include ACE inhibitors, beta-blockers and calcium-channel blockers. 

The Mayo Clinic explains that with inner ear disorders your brain receives signals from the inner ear that aren’t consistent with what your eyes and sensory nerves are receiving. Vertigo may result as the brain works to try and sort out the confusion. One common inner ear problem is benign paroxysmal positional vertigo (BPPV). The condition comes on with sudden movements of the head that cause vertigo and is treated with certain maneuvers and medications like meclizine (an antihistamine used to treat motion sickness and dizziness).

Meniere’s disease is another inner ear condition and is the result of a fluid built-up in the inner ear. Symptoms include long-term vertigo caused by things like too much salt in the diet, coffee, alcohol and stress, as well as viral infections and congestion. Treatment includes altering your diet and medications to relieve fluid build-up.    

Dizziness can also be caused by migraines, neurological conditions, dehydration, low blood sugar and other conditions. It is important to remember that dizziness can increase your risk for a fall. If the condition persists, talk to your doctor who can help narrow down the cause of your dizziness and prescribe the appropriate treatment.

Healthy Eating Supports Healthy Aging

The National Institute on Aging (NIA) explains diet plays a large role in the aging process. To optimize our own health, they recommend we start by understanding the different food groups (published by the USDA); and how each one helps form healthy eating patterns for a particular age group. 

Experts say diet is one aspect of healthy aging and is especially important for seniors who are at greater risk for cardiovascular disease, cancer and diabetes as well as conditions related to loss of bone and muscle mass such as osteoporosis and sarcopenia. Other aspects of healthy aging include maintaining a healthy weight, exercising, and getting a good night’s sleep. 

One of the best food groups that encourage healthy aging is vegetables, which are rich in vitamins and minerals, and carbohydrates. These include dark green vegetables, red and orange vegetables, and starchy vegetables; as well as legumes (beans and peas) that are also found in the protein food group.  

The NIA reports fruits are another critical food group and are not consumed often enough by older adults. Fruits contain carbohydrates along with extra fiber that helps keep the digestive system moving. Recommendations include eating fruits with the skin on but be sure to wash them thoroughly before eating. If you choose to eat fruit in a can, dried or from frozen, choose options lowest in sugar.  

Fiber, a type of carbohydrate the body doesn’t digest, is another healthy food group. It can be found in vegetables, fruits, whole grains, and beans. Fiber-rich foods can help prevent stomach or intestinal problems and lower cholesterol.

At least half of our daily intake of grains should be consumed in products made with whole grains. These provide iron and many B vitamins. Examples include whole wheat bread, whole oats, whole bulgur (also known as cracked wheat), and whole cornmeal. 

For protein, it is recommended we choose low-fat meats, poultry, seafood, eggs, beans, nuts, seeds, and soy products. Dairy products help maintain strong bones and provide several vital nutrients including calcium, potassium, and vitamin D. Again, the best choices are low-fat options. Experts also say to use oils instead of solid fats, such as butter or lard that are high in saturated fat, whenever possible.

Foods You Don’t Need to Wash before You Eat

If you have concerns about food safety, and many of us do, it can be confusing on what foods needs to be washed before eaten. The US Department of Agriculture (USDA) weighs in on the issue regarding foods that do not need to be washed at home, and in some cases, may be detrimental to do so.

With all of the media attention regarding bird flu, some people wonder if we should be washing our store-bought eggs. According to the USDA, we should not wash eggs from the grocery store as commercial eggs are already washed. Experts explain,” Bloom,” the natural coating on just-laid eggs that helps prevent bacteria from permeating the shell, is removed by the commercial washing process. It is replaced by a light coating of edible mineral oil, which restores protection for long-term home storage of eggs.”  If fact they warn that washing eggs at home could increase the risk of cross-contamination, especially if the shell becomes cracked. 

Another common food we buy is pre-washed salad mixes. Again, the USDA says it is not necessary to wash pre-washed or ready-to-eat produce as they are safe to eat as packaged. If you do decide to wash it, they warn about cross-contamination and to take care to avoid unclean surfaces or utensils that have come into contact with raw meat, poultry or seafood, or their juices. A tip for washing fresh fruits and vegetables is to hold them under running tap water to remove any dirt. For veggies or fruits with a firm skin, scrub with a clean brush. Avoid using detergent soaps or commercial produce washes.  

Most canned foods do not need to be washed. However, when using canned beans; follow the recipe recommendation in terms of rinsing. Rinsing rice before cooking can help reduce rice’s arsenic content as well as remove the surface starch for a lighter, fluffier rice. If you are cooking risottos, paella or rice pudding, skip the rinsing step as you want the extra surface starch to remain on the rice. Quinoa from the supermarket is typically already rinsed and dried before packaged. Check the label to see if it says pre-rinsed or if it recommends rinsing.
