The Connection between Our Immune System & Aging

Our immune system is similar to an engine. As we age, our parts start to slow down and show wear and tear. The body no longer runs as smoothly or as robust as it used to, making it harder to fight off infections. Growing old makes it more difficult to recover from strenuous activity or bounce back from illness. A recent article in the US News & World Report likens it to an older smart phone not holding its charge as well as it once did. 

Experts explain the immune system is made up of two parts referred to as innate immunity and adaptive immunity. Our innate immune system is the body’s faster response to pathogens and relies on physical barriers like the skin or mucous membranes, and special cells that digest germs and/or destroy infected cells. Our adaptive immune system is responsible for a slower, more specific defense against pathogens using antibody B cells to destroy the pathogens; or T cells to coordinate and kill infected cells. 

Dr. Austin Perlmutter, a Seattle-based, board-certified internal medicine physician explains aging brings about many changes. He states, “The most prominent of them is the development of what is called ‘immunosenescence’, a progressive state of worsening of the overall immune system function.” Not only do the elderly have fewer immune cells but the ones they have are fairly worn out. This means seniors are more likely to have chronic inflammation as well as a weaker response to infections; and unfortunately, the protections vaccines offer.  

Experts say while aging inevitably affects our immune system, we can do things to support and strengthen it throughout our lives such as getting routine vaccinations and following a healthy lifestyle. Managing chronic conditions additionally helps reduce the risk of severe infections and help us recover quicker.

The National Council on Aging reports that just as regular preventive medical screenings, vaccinations and dental exams are vital to maintaining our best health, so is protecting our social and mental health. Studies show older adults who say they feel isolated are affected in several negative ways. 

Researchers explain there is a connection between those who say they have a good social life with a longer life span. Studies indicate it may be because these are people who appear to handle stress better; something that is linked to cardiovascular disease.

Studies also find seniors who report having a better quality social life were more likely to regularly exercise. Another concerning finding is that isolation is linked to an increase in a person’s risk for anxiety and depression, which are both linked to a higher risk for dementia.  

Many seniors living alone have lost a spouse or other close family members and friends and feel very alone. Experts say if this is your situation, they suggest checking with your local senior center, church or library about interesting activities they may sponsor; or look into joining a book club, gardening club or other hobby groups. If you are an animal lover, think about adopting a pet as they provide companionship and a sense of purpose. Shoulder Blade Pain

The shoulder blade, referred to as the scapulae, consists of triangular bones on the back of the shoulders; and are attached to the top of the arm at the shoulder joint and the clavicle (at the acromioclavicular joint). As the area is surrounded by muscles, tendons and ligaments, doctors say determining the cause of shoulder blade pain can be difficult. 

The Medical News Today website explains shoulder blade pain can be caused by a number of conditions including a heart attack. Other causes may be sleeping in an uncomfortable position, strain from lifting a heavy object, and physical trauma. 

Examples of bone or joint problems are pain caused by a fall or accident that results in a fracture. Other bone and joint issues include degenerative conditions like arthritis, bursitis or frozen shoulders. One type of arthritis called ankylosing spondylitis primarily affects the spine and can cause pain and inflammation in the surrounding area including the shoulders. 

Shoulder pain that arises beneath the bone that forms the “roof” of the shoulder (the acromion) is often referred to as shoulder impingement. Experts say the cause is often unknown and in about 50% of people, the pain gets better after around 6 months. Treatment includes pain medication, cooling the area and physical therapy. 

Muscle and tendon problems causing shoulder pain include the rotator cuff, muscle strain and overuse of the area. The rotator cuff is a set of muscles that helps the arm and shoulder move and injuries are common including tears to one of the cuff tendons. Treatment includes taking over-the-counter nonsteroidal anti-inflammatory drugs or pain medications to minimize swelling and discomfort, applying ice packs and physical therapy. In some cases your doctor may recommend surgery. 

Consult your healthcare provider about lingering shoulder blade pain. Also, remember shoulder pain is associated with a heart attack, so anyone experiencing shoulder pains along with shortness of breath, or other signs of a heart attack, should seek immediate medical attention.

Experts say it is important to remember you are not alone. Over six million people in the US are living with Alzheimer’s and millions more know someone with the disease. Although everyone experiences the disease in their own unique way, understanding the major stages of the disease helps families navigate this unexpected journey. 

Alzheimer’s five distinct stages include: Mild cognitive impairment (MCI); Mild; Moderate; Severe; and Very Severe. Symptoms can vary from person to person but the various stages of Alzheimer’s disease have some common characteristics and it helps to be prepared.

Longevity Secrets from Centenarians

According to recent statistics, there are an estimated 101,000 centenarians (age 100+) making up 0.03% of the population; and experts expect that number to quadruple over the next three decades. Compare that to just 2,300 centenarians estimated to be alive during the 1950 census. Interestingly, Japan has the greatest number of people in their 100s, at 146,000 with the US holding the second spot. China has (60,000), India (48,000) and Thailand (38,000) to round out the top five.

Experts say staying physically active is beneficial for overall fitness as well as extending our lifespan. Many centenarians report they routinely engage in physical activity such as walking, gardening, or participating in community activities. Studies link staying active to maintaining strength, flexibility, and cognitive function, which all contribute to not only a longer life but a healthier one.

To learn the secrets to a long life, Today.com interviewed several including some that are still driving, working and even getting married. One 100 year old recommends not retiring if you are happy working. In fact she reports to work every day at her family’s furniture store. The woman still drives, and once she is home, cooks her meals from scratch of which many of the vegetables and herbs come from her own garden. In terms of diet, she says she doesn’t eat fried or fast food, avoids alcohol and makes getting a good night’s sleep a priority. 

Another 102 year old woman passes along advice that includes the philosophy of having “no regrets,” and recommends not wasting time looking back but rather looking forward to the things you want. She still works three days a week and her health routine includes walking every day and staying positive.   

Others interviewees said to eat lots of fruits and vegetables, and avoid foods high in sugar. There were many who commented on the benefits of a positive attitude, and said they try to bring joy and love into their lives. The centenarians also agreed expressing gratitude and spending time with loved ones is important to a fulfilling, long life.

Age Wave, a company specializing in issues related to the aging population recently surveyed how older adults feel about senior living. It examined the decision-making process including priorities, hopes, concerns, and questions of those surveyed as well as some of the interviews.  

The first topic the survey looked at is whether a senior’s current home is in reality the best place to retire in. Age Wave said results show a move to senior living can in many cases provide more freedom, more convenience, better care and less worry for seniors. 

One sentiment expressed by many in retirement communities is that they appreciate the fact that if their health needs change, or those of their spouse, the community is equipped to deal with the needs without necessitating a move. Also if they need to see a doctor, they like that they can just schedule a ride to their appointment. 

Another myth is people fear they will no longer have purpose in their life in a senior community. However, according to the Age Wave survey, people in CCRC reported at this time of life perhaps there is a better purpose than doing household chores. Residents expressed their satisfaction in the freedom to explore new pathways – “from learning how to paint, volunteering in the community, becoming an expert on healthy cooking, authoring a book, reaching new levels of fitness, to developing rewarding new friendships.”

Happiness Has Positive Effects on Health and Well-Being

Happiness not only makes us feel good but may also be good for us. Research shows happiness is an important component to maintaining physical health, and is linked to lowering the risk of stroke, high blood pressure and cardiovascular disease. Along with better health, people, who report they are happy, tend to live longer. 

Being happy can help us eat better, maintain a normal body weight and allow us to get a better night’s sleep. It can encourage people to be more active and reduce stress.  In fact, there is an understanding that the mind-body connection is stronger than we thought and unmanaged stress can lead to or exacerbate many health conditions. 

Don’t feel like you are all that happy? No problem. Experts say there are things we can do to cultivate happiness, and improve our outlook on life as well as our physical health. Some of these things include forming and maintaining uplifting connections, practicing gratitude and mindfulness, and taking the time to appreciate the beautiful moments of life. 

Expressing gratitude has been shown to be an excellent deterrent to depression and anxiety. Dr. Robert Emmons, a renowned researcher in the science of gratitude, says, “Gratitude is literally one of the few things that can measurably change people’s lives.” He adds it encourages feelings of joy, love and optimism and reduces feelings of envy and resentment. One way to cultivate gratitude is to daily journal at least one thing you are grateful for as writing it down appears to reinforce your feelings.

The practice of mindfulness is another way to bring more happiness into our life. It helps cultivate a deeper connection with our inner self, and improves mental health by reducing stress and anxiety. The National Institute of Health explains practicing mindfulness is as easy as breathing through your nose to a count of 4, holding for 1 second and then exhaling through the mouth to a count of 5. Repeat often. The Internet has other techniques for mindfulness practices.

Healthy Lifestyle Tips for MCI Management

Mild cognitive impairment (MCI) can be a precursor to dementia but does not always progress to dementia. Medical professionals report 10% to 40% of MCI cases go on to become dementia, and a good percentage of those become Alzheimer’s disease. On the positive side, research shows as many as 30-40% of people with MCI get better. As with Alzheimer’s disease and other types of dementia, there is no cure for MCI but there are some therapies which have been approved to treat early Alzheimer’s to slow disease progression.  Doctors also say there are steps we can take to mitigate or minimize the effects of MCI. 

MCI is just as its name suggests. The impairment is mild and does not affect a person’s ability to care for themselves or take away their independence. Steps a person should take to protect their cognitive health include quitting smoking and moderating or eliminating alcohol consumption. If you do have hypertension (high blood pressure), high cholesterol levels or diabetes, it is important to treat these conditions and manage them through medications, diet and healthy lifestyle habits. 

Other things you can do to thwart, delay or minimize the effects of MCI are to maintain a healthy weight and exercise, which is good for our brains as well as our bodies. Avoid junk food, highly processed foods and foods high in sodium; and reduce the amount of red meat in your diet. Isolation has also been linked to a higher risk for dementia, so it is important to stay social.

However just like other healthy habits; the practice of gratitude takes a conscious effort. Experts say to receive the full benefits; it should not be saved only for special occasions but rather practiced every day. 

To begin, we need to understand exactly what “gratitude” is. Erin Wiley, M.A., L.P.C.C., a licensed clinical psychotherapist and executive director of The Willow Center, explains that gratitude is “a positive state of mind evoked by focusing on and appreciating the good in one’s life.” By adopting a positive state of mind, we can concentrate on the good things as well as deal with negative events that come into our lives in a more fruitful way.  

A helpful suggestion for getting into the habit of feeling more grateful includes using a journal daily to express your gratitude. Take the Time for Self-Care

Most of us experience some level of stress whether it is just occasionally or something that bothers us on a more frequent basis. If you want to know how to handle stress better, Dr. Shalu Ramchandani, an integrative medicine specialist at the Harvard-affiliated Benson-Henry Institute for Mind Body Medicine at Massachusetts General Hospital, recommends following good health habits. These include eating a predominantly plant-based diet, exercising on a regular basis, sleeping at least seven hours a night and staying socially connected. 

Another beneficial habit to reduce stress is to practice self-care, which is simply taking time out of your day to do something that makes you feel good. Experts recommend one of the top self-care habits is to practice mindfulness. Spending too much time on things such as problem-solving and negative thoughts can make you more likely to experience stress, anxiety and symptoms of depression. Practicing mindfulness with breathing exercises, guided imagery and other practices can help you direct your attention away from this kind of thinking and relax the body and mind.

Self-care habits can include taking a walk out in nature, practicing yoga, taking a bath or doing some stretches before bedtime. Some people feel a sense of calm using aromatherapy with scents known to be calming like lavender, rose, bergamot and frankincense. Lighting a room using candles or getting a massage can also be effective. 

Research finds watching a movie or an episode of a comedy show that makes us laugh can additionally reduce our stress hormones. For others, listening to soothing music may be calming. If coffee or caffeinated drinks make you feel jittery or anxious, cut back and replace with decaffeinated substitutes or drink water or natural fruit juices. If nothing seems to help, and your stress and anxiety levels are not improving, consult your healthcare provider.

While there is not one single test used to definitively diagnose Alzheimer’s disease, doctors typically use a variety of tools to make a diagnosis. Along with the person’s primary care provider, specialists like neurologists, neuropsychologists, geriatricians and geriatric psychiatrists may all be involved in determining if the patient’s problems are Alzheimer’s or caused by other reasons.

The process starts with a medical workup done by a health care provider that includes the person’s history of medical problems as well as any history of psychiatric, cognitive or behavioral problems. It is important to tell the doctor about all current medications including over-the-counter-drugs and supplements the person is taking. Your doctor will also want to know if there is a family history of Alzheimer’s or other cognitive problems. 

A physical exam should include a check of reflexes, blood pressure levels and questions regarding sleep patterns, diet, exercise and other lifestyle habits. A structural brain imaging scan such an MRI or computed tomography is used to rule out damage to the brain from stroke, tumors, head trauma or a build-up of fluid. Blood and urine tests can rule out other reasons for symptoms. The Alzheimer’s Association says common causes of dementia-like symptoms include depression, untreated sleep apnea, delirium, side effects of medications, thyroid problems, certain vitamin deficiencies and excessive alcohol consumption.

The doctor may also use one of several cognitive, functional tests to evaluate the person’s memory, thinking and simple problem-solving skills. Professionals state, “There is an urgent need for a simple, inexpensive, non-invasive and easily available diagnostic tool such as blood tests to diagnose the disease.” Currently researchers are experimenting with a variety of tests to enable them to track changes more easily in the brain to identify the disease. Health professionals do not recommend routine genetic testing for Alzheimer’s disease at this time.

Getting Enough Vitamin D

There has been much in the news lately about Vitamin D including how much we need each day and the fact that far too many people may be deficient. Medical professionals say there are not many foods high in Vitamin D so relying only on what we eat can be challenging. It is estimated close to half of all Americans are deficient in D; but without a blood test, it is difficult to know for sure. 

Vitamin D is good for us as it contributes to bone health by aiding our bodies with absorbing calcium and phosphorus. It additionally helps nerves carry messages between the brain and other parts of the body as well as fights inflammation.  

A food high in D is rainbow trout. Dieticians say one 3-ounce serving contains 645 IUs, which is more than the daily recommended serving for adults. Other foods high in D include salmon, which is high in vitamin D and omega-3 fatty acids, egg yolks and canned tuna. Mushrooms grown under UV light also have a good amount of vitamin D. Another source is food fortified with D such as many types of milk, yogurt, cereals and orange juice. 

If you are concerned about sun exposure due to the risk of skin cancer and don’t want to get vitamin D that way, experts say it only takes about 10 to 30 minutes of daily direct sunlight exposure to ensure vitamin production. The National Institutes of Health recommends a daily allowance of 600 international units IU (15 micrograms) of vitamin D for most adults. For seniors 70 years and older, 800 IU (20 micrograms) is the daily recommendation. Ask your doctor for a blood test to determine your current levels and how much vitamin D you need.

When is it Time for Cataract Surgery?

Statistics show cataracts affect approximately 24 million Americans and are the leading cause of blindness throughout the world. They occur when the eye’s natural lens becomes cloudy due to the breakdown of its proteins, resulting in blurry, hazy or less colorful vision. Age is the primary reason for cataracts and typically starts around the age of 60. As they take a long time to develop; many people do not notice the gradual blurring of their vision. Individuals with diabetes may experience the problem at a much younger age. 

If during an eye exam your doctor mentions you have the start of cataracts, they may tell you it is not to the point where surgery is required; but may recommend increasing the strength of your eye prescription. However if you are told the problem is far enough along to warrant surgery keep in mind it is a fairly routine procedure but do talk to your doctor about the benefits and risks to determine if it is the right time for you. 

There are a few signs that may make it easier to decide if you want to do the surgery. These include having a hard time driving at night due to cloudy vision and not being able to read as well or watch TV because of the cataracts. Double-vision and seeing colors as faded or yellowish are other signs. Another reason it may be time to schedule the surgery is the cataracts are making it difficult for your doctor to do a thorough exam of your eyes. 

One of the best ways to slow down cataracts is to wear sunglasses with UV protection and to quit smoking. If you see “halos” or can’t handle glare while driving at night, stop driving after dark. Experts say cataract surgery has a high success rate but it can take anywhere from two to six weeks to fully recover.

Helping Slow Cognitive Decline

An article in Today’s Geriatrics Medicine states many adults as early as their 60s start to experience a slip in their mental abilities. By the age of seventy, studies show as many as 67 percent of people experience some level of cognitive decline. A small group of those people (around 10 percent) will go on to develop more serious cognitive diseases such as Alzheimer’s. However is it good to know not everyone who is diagnosed with MCI (mild cognitive impairment) will progress to Alzheimer’s.

We know the risk for Alzheimer’s is higher in those with a genetic connection or have a history of head trauma or cerebral ischemia. Modifiable factors including lifestyle habits like alcohol use, smoking, poor nutrition and obesity can also increase one’s risk for dementia. Researchers say in the early stages of cognitive decline improving things like “diet, stress management, sleep quality, mental health, social connections and exposure to toxins” may help mitigate or prevent further decline. Other problems that may interfere with cognition are chronic systemic inflammation, hormonal imbalances, low levels of vitamin D and insulin resistance. 

People in the early stages of cognitive decline should be assessed for nutrient deficiencies and underlying health conditions that may accelerate their condition; and be provided information on specific foods and nutrients known to support brain health. For those with Alzheimer’s disease, experts recommend caregivers and dietitians encourage a structured and enjoyable eating experience formed around the individual’s eating preferences and cultural traditions. It is important to remember you can’t force someone to eat something they don’t like just because it is good for them.

Aging and Forgetfulness

The National Institute on Aging (NIA) reminds older adults mild forgetfulness is more often a normal part of aging and not a serious memory problem. They explain serious problems generally involving things like the person asking the same question over and over again and getting lost in places they know well. They may also have trouble following recipes or directions and find themselves confused about people, places and time. 

If you are someone who is forgetting things, there are some steps you can take to feel more confident. These involve several simple things you can do, which will benefit your whole being like getting enough sleep (seven to eight hours a night), not drinking a lot of alcohol; and exercising and eating well. Managing high blood pressure is also important.

Other more practical tips to help with forgetfulness are putting your wallet or purse, keys, glasses and phone in the same place each day. Instead of assuming you will remember, write out shopping and need to-do lists and use a calendar or take notes. NIA says to get involved in activities that focus on using the mind and body and stay socially involved. If you feel depressed for weeks at a time, talk to your doctor. 

Strategies to improve forgetfulness include challenging our brains by learning new things as well as using all our senses. For example, prepare a new recipe that requires lots of spices. If you are eating out, try to identify the flavors and seasonings used in the meal. To help remember new information better such as names, places, etc., try repeating it out loud or writing it down as this reinforces the memory. For example, if you want to remember a new neighbor’s name, say it out loud back to them, like “hi Jack.” Then an hour or so later say the new neighbor’s name out loud again.
