Reducing the Risk of a Fall

The Center for Disease Control & Prevention (CDC) reports millions of older adults fall each year—resulting in more than 36,000 deaths. Of those falls, 3 million seniors are treated in emergency departments for fall injuries; and one out of every five falls causes an injury, such as broken bones or a head injury. With these alarming statistics, experts say it is important for seniors to know their risk for a fall including whether they are taking medications associated with an increased risk. 

The first step is to talk to your doctor to see if you are taking one of these medications, as medications are a common reason for falls. Furthermore, experts say adjusting or making a change in medication is the easiest thing to do to help prevent a fall. If it is determined you are taking a medication known to cause light-headedness, dizziness, sudden drop in blood pressure, or chronically low blood pressure or hypoglycemia (low blood sugar), you and your healthcare team can regularly re-evaluate your need and dosage. 

Another way to mitigate your risk for a fall is to do exercises designed to improve leg strength and balance. In addition, be sure to get an annual eye exam and update your glasses as needed. Lastly check your home for trip hazards (keep clutter out of hallways and stairways) and install grab bars and better lighting where needed.

If You Are Experiencing Memory Problems

Experts explain the more we know about urinary tract infections, the better we can protect ourselves from getting one as well as quickly getting proper treatment when we do have one. According to a 2019 report from Therapeutic Advances in Urology, 50% to 60% of women experience a UTI, defined as an overgrowth of bacteria in the urinary tract, at least once in their lifetime. 

Understanding the urinary tract is a good place to start in understanding the infection. The lower urinary tract includes the urethra, the tube allowing urine to exit the body; and the bladder, which stores and empties the urine through the urethra. The upper urinary tract includes the ureters that carry the urine from the kidneys to the bladder; and the kidneys making the urine.  

UTIs are more common in the lower urinary tract and symptoms include a change in the color and smell of urine. It may also appear cloudy and be dark or red looking, and smell bad. Many feel as if they are unable to completely empty their bladder and may experience pain when urinating. In some cases people experience fever and chills, pain in the pelvic region and a need to urinate more frequently.   

Infections in the upper urinary tract involve the kidneys, and while less common can be more severe. A person may have a fever above 102.5 F, blood in the urine and flu-like symptoms. 

Frequent UTIs in women can result from bladder prolapse, kidney stones and being post-menopausal. Although UTIs are more common in women, doctors say men do get them; and more often as they age. Many older men suffer from an enlarged prostate, making it harder to completely empty the bladder and setting up an environment for bacteria to develop in the remaining urine. 

Doctors say there are some steps we can take to help prevent UTIs including staying hydrated during the day and urinating when you have the urge. Other self-care for women includes urinating after sex and being careful to wipe from front to back. Medical professionals say avoiding douching and using powders and sprays in the genital area as much as possible. If you have diabetes, it is important to manage the disease as best as possible for many reasons including being more susceptible to UTIs.

The Alzheimer’s Association explains, “Physicians and other medical professionals are a central part of the care team of a person living with dementia. Along with other members — which may include family, friends or others — they work to provide support throughout the course of the disease.” Therefore, one of the best ways to ensure optimal care is to honestly communicate with your physician.  

To establish a good relationship with the doctor, a person or family member should take some time to prepare for the visit. Helpful tips from the Alzheimer’s Association include:

  • Document any changes in your health, including your mood, memory and behaviors.

  • Make a note of any changes you’ve noticed since your last appointment.

  • Create a list of current prescriptions and over-the-counter medications.

  • Write down any vitamins or supplements you’ve added to your medication regimen.

  • Make a list of questions you want to ask.

Writing down questions you have beforehand makes sure you don’t forget them, and don’t be reluctant to ask questions if you don’t understand something. The Alzheimer’s Association National Early-Stage Advisory Group, made up of men and women in the early stages of Alzheimer’s or other forms of dementia, put together a list of questions to help others make the most out of a doctor’s visit. Questions range from asking about the type of tests used to make a diagnosis, what to expect from the disease, and what type of treatments may be used. When seeing a specialist, ask how familiar they are with caring for patients with Alzheimer’s disease.

Harvard Health restates a classic text that says tai chi will help you become “Strong as an oak, flexible as a willow, and [mentally] clear as still water.” The ancient Chinese tradition is based in the martial arts; and is a popular low-impact exercise involving slow movements and breath. Experts say tai chi is suitable for people of all ages including seniors and offers numerous benefits.  

Tai chi’s mind-body exercises consist of slow, flowing, and choreographed movements that provide many people a sense of calm, which is why it is often referred to as mediation in motion, or medication in motion. Its movements focus on control as well as slowing down and relaxing muscles to allow the body to move more freely. Harvard Health says for those in their 60s, 70, 80s and beyond, “tai chi’s restorative powers can help you age more gracefully and function at a higher level than those who don’t practice.” 

Other known benefits include offering relief from common ailments such back, neck and arthritis pain, and fibromyalgia as well as reducing the risk of heart disease. One study reports older adults who practiced tai chi one to three times a week had better balance and were 43% less likely to fall, and cut their risk of injury in half. In addition, research suggests tai chi improves mood and is an effective treatment for depression. It may also help reduce age-related cognitive decline and slow dementia.

Is there Any Benefit From Taking Multivitamins?

As we age, it becomes more important than ever to protect our health but making decisions on how we do it can be confusing at best. One day it seems this diet is better than another and they have lots of initials such as DASH, MIND, TLC or KETO as well as Mediterranean, vegan, vegetarian and flexitarian diets. Adding to the challenge is whether we need a daily multivitamin. 

While it is a good idea to get advice from your healthcare provider, it doesn’t hurt to hear what some other experts say. For example, the Harvard School of Public Health says if you eat a healthy diet, a multivitamin may have little or no benefit. A healthy diet should include plenty of vegetables, fruits, whole grains, good protein sources and healthy fats. 

However the National Academy of Medicine cautions some Americans may not be getting enough of specific vitamins and minerals such as vitamin D and vitamin E from food sources alone. Vitamin deficiency can also occur in people with diseases like celiac, ulcerative colitis, or cystic fibrosis, which cause their digestive systems to not function properly. Others who may benefit from the addition of specific vitamins include the elderly or conditions such as pregnancy, alcoholism and taking certain medications including treating high blood pressure. 

Doctors say even those with diseases such as osteoporosis may only need to increase their intake of nutrients targeted toward maintaining bone health, or if that is not possible to take a supplement. If in doubt, ask your healthcare team if you should consult with a dietician. Remember to let your doctors know about any supplements you are taking in case of drug interactions with prescription medications.

Research on the Benefits of Walking for Exercise

Over the last several years, reports on the “optimal” number of steps we should take each day seem to vary. Early on it was all about getting 10,000 steps but the Medical News Today’s website explains the idea of 10,000 steps was conceived in the 1960s as a marketing tool and was not based on any actual medical research.   

A study from the Vanderbilt University Medical Center in Nashville, TN, decided to take a “fresh look at how many steps a person should take each day — and what kind of steps are most beneficial — to promote good health.” The data was obtained with fitness trackers and the information led researchers to suggest a goal of 8,200 steps a day significantly lowers a person’s risk of chronic disease. 

Researchers report results show an association between walking 8,200 steps and a reduced risk of chronic conditions, including:

  • obesity

  • sleep apnea

  • gastroesophageal reflux disease (GERD)

  • major depressive disorder (MDD)

  • diabetes

  • hypertension

In addition, they found even more steps a day could reduce the risk even more for some of these diseases. However, Dr. Evan L. Brittain, associate professor of medicine at Vanderbilt, explained, in the case of “diabetes and hypertension, we observed a plateau at around 8–9000 steps per day, above which there wasn’t any obvious benefit.” He also reminded people there is no reason to not get in more steps a day as they can be beneficial for other health conditions.
