Preparing for a Move to Senior Living Eases the Transition

Moving can be exhausting and many of us have mixed feelings about the change as we look to a new adventure, while sad to leave our old life behind. Now imagine what your parents may be feeling as they leave a familiar home for an assisted living community. The change can be even more difficult if they are going into memory care. 

Experts say there are things family members can do to ease the transition and make it a more positive experience. First, involve your loved one into the decision-making process as much as possible. Also read online reviews from other families and residents; and take advantage of in-person tours.

A 2023 survey from the National Poll on Healthy Aging showed one in three adults aged 50–80 (34%) felt isolated from others (29% some of the time, 5% often) in the past year. 

Experts say this can be problematic as research shows older adults who express loneliness have a 32% increase in the risk of stroke and a 29% increase in heart disease. Other problems include a disruption in sleep, higher blood pressure and increased stress levels. It can also be a predictor of functional decline and even death in adults over 60. WebMD cites studies linking people who are socially isolated to an approximately 50% higher risk of getting dementia.

As you grow older, children may have moved away or a spouse died, and you may have experienced the loss of other close relatives and friends. You may no longer work, and thus no longer enjoy companionable relationships with work colleagues. Vitamin D, How Much Do You Need and How to Get It

There has been much in the news lately about Vitamin D including how much we need each day and the fact that far too many people may be deficient. Medical professionals say there are not many foods high in Vitamin D so relying only on what we eat can be challenging. It is estimated close to half of all Americans are deficient in D; but without a blood test, it is difficult to know for sure. 

Vitamin D is good for us as it contributes to bone health by aiding our bodies with absorbing calcium and phosphorus. It additionally helps nerves carry messages between the brain and other parts of the body as well as fights inflammation.  

A food high in D is rainbow trout. Dieticians say one 3-ounce serving contains 645 IUs, which is more than the daily recommended serving for adults. Other foods high in D include salmon, which is high in vitamin D and omega-3 fatty acids, egg yolks and canned tuna. Mushrooms grown under UV light also have a good amount of vitamin D. Another source is food fortified with D such as many types of milk, yogurt, cereals and orange juice. 

If you are concerned about sun exposure due to the risk of skin cancer and don’t want to get vitamin D that way, experts say it only takes about 10 to 30 minutes of daily direct sunlight exposure to ensure vitamin production. The National Institutes of Health recommends a daily allowance of 600 international units IU (15 micrograms) of vitamin D for most adults. For seniors 70 years and older, 800 IU (20 micrograms) is the daily recommendation. Ask your doctor for a blood test to determine if and how much vitamin D you need.

Cognitive Reserve and Its Importance to Brain Health

While the concept of cognitive reserve was identified three decades ago, researchers were not sure how to quantify or separate it from the term brain reserve, which is defined as a structural quality measured by brain volume and its number of neurons and synapses. The NIH says to refer to it as actual differences in the brain itself that may increase tolerance of pathology, and cognitive reserve.

The term cognitive reserve was first used by a group of neuropsychologists at Columbia University lead by Yaakov Stern to explain why people of higher education or in more challenging occupations were less likely to develop Alzheimer’s disease. After examining the brains post mortem of elderly residents from a care facility, they found some of those with a high degree of plaques associated with Alzheimer’s had “ranked in the 20 percent of residents as good or better than those without any signs of plaques.” They hypothesized the extra years of intellectual effort gave people a reserve to help delay the onset of clinical manifestations.  

New evidence debunks the long-held view that brain reserve is a fixed capacity throughout one’s life and finds we may be able to make new neurons through adulthood. However scientists say this ability needs to be supported by cognitive resilience or “brain maintenance” consisting of lifestyle or environmental factors to ensure the brain’s well-maintained. Therefore optimal brain health is a combination of cognitive reserve, brain reserve and brain maintenance. 

While cognitive reserve is associated with higher IQs, higher levels of social engagement in mid or later life is also an important component of cognitive performance. In fact studies show older adults with strong social ties have a 30 to 50 percent lower dementia risk. Exercise also elevates blood flow to the brain and maintains health. Risk factors impacting our reserve are obesity, lack of sleep and poor diet. Scientists explain building and protecting cognitive reserves is about exercising our brains throughout our lives with activities that include physical, mental and social stimulation. 

Researchers explain the physical manifestation of Alzheimer’s in the brain is linked to loss of synapses; and a recent discovery of a neuritin (NRN1), may provide an answer to therapeutic intervention. Jeremy Herskowitz, a researcher from the University of Alabama, says one day NRN1 could be a supplement to support brain function and help forestall the pathological effects of Alzheimer’s disease.

Screenings Guidelines for Women

Medical professionals stress the importance of women getting the proper screening to protect their health and discover problems early when they may be more treatable. Screenings are designed for otherwise healthy individuals and recommendations change as we age. The publication US News & World Report, states, “Your doctor can help you tailor the recommendations based on your goals of care, personal and family health history, age and life expectancy.”

One of the most recommended screenings for women over the age of 40 is to have an annual mammogram. Doctors say mammograms are key to the prevention as well as early detection of breast cancer when it is most easily treated. Talk to your doctor about testing options if you are under the age of 40 and have a mother or sister who contracted breast cancer at a young age. 

It is recommended women 50 years and older have a colonoscopy every ten years. As colon cancer is more common in African-Americans and occurs at an earlier age, it is advised to start testing at age 45. Younger women/teens with a family history of adenomatous polyposis – a genetic condition that starts in someone as young as in the teen years may need colonoscopies at a much earlier age. Also a woman with a history of Crohn’s disease or inflammatory bowel disease will likely be advised to have a colonoscopy at a younger age than 50.    

Diabetes is another disease a woman 45 years and older should be screened for every three years. Exceptions for screening earlier are a family history of diabetes; obesity and high blood pressure readings or you take medications for HBP.

A Lipid profile assessment testing a woman’s cholesterol and triglyceride levels as well as the risk for heart disease should be done every five years starting at age 45. If lifestyle changes do not reduce your numbers, you may need medications. As with all screening, discuss with your doctor how often you need to be screened and what steps to take to improve your health.

Walking Is One of the Simplest Ways to Improve Balance

Many seniors know a fall could threaten their independence and way of life. Common injuries include to the head, hip fractures, and in the most extreme cases death. Experts explain balance declines as we age as well as can be affected by medications and medical conditions, lack of flexibility and uncorrected vision problems. This is why medical experts encourage seniors to work on maintaining and improving balance. 

The Harvard Medical School says one of the simplest ways to improve balance is with exercises like walking, strength training and workouts targeted to improve stability. However, before beginning any new exercise regime, consult your healthcare provider.  

When beginning a walking plan, whether you are active or sedentary, experts say to remember minutes walked are more important than number of miles. If you are unused to exercise, start slowly. Also, wear good walking shoes, and if you normally use a cane or walker, use it. Experts say it is safer to walk with a buddy and to bring along a cell phone. Walk in a place with an even surface, and especially in the beginning, stay close to home.  Once you start feeling more confident, add more minutes to your routine. 

For seniors who are already walking, try adding a few more minutes to your program or a more challenging route. Experts say the overall goal should be to do at least 150 minutes a week. To add strength training exercises and workouts to improve balance, ask your doctor for advice on how to get started.

With a Little Effort, We can be Our “Best Self” as We Age

Medical experts discuss the natural changes and not so natural changes our bodies go through as we age, and things we can do to help mitigate or delay some of the impact. The first thing to consider is that our heart must work harder as our arteries and blood vessels become stiffer, leading to things like high blood pressure and other forms of heart disease. 

To keep your heart as healthy as possible, doctors recommend staying active each day. Walking, swimming or even moderate types of exercise can help maintain your weight. A heart-healthy lifestyle also includes sleeping 7 or 8 hours a night and eating a diet rich in vegetables, fruits, whole grains and plant proteins.

Loss of muscle mass can reduce body strength and make it difficult to get around safely. Experts suggest daily exercise like a brisk walk and/or lifting light weights can help improve strength and function. Remember it is always important to check with your doctor to see how much exercise is right for you. 

Older people may begin to notice changes to their sight and hearing. You may find yourself needing reading glasses or having difficulty driving at night. A sign of hearing problems can include trouble hearing higher frequencies, needing to increase the volume on the TV, or following conversations in a crowded restaurant. The best remedy is to have your vision and hearing regularly checked; wear sunglasses when in the sun, and earplugs to protect from loud noises.  

Occurrence of tooth decay, plaque, infections and gum disease can be more frequent as we age. Good dental health includes brushing and flossing twice daily and getting regular check-ups and teeth cleaning. 

As our bones begin to weaken in our 40s, and bone loss is sometimes measured by a decrease of an inch or two in stature; joints can start to feel stiffer as fluid and cartilage decreases and wears away. The 50s and 60s can bring on the first signs of arthritis, which is why doctors stress it is important to get enough calcium in your diet and vitamin D.

We all have occasional aches and pains but when problems persist or interfere with daily life, it is time to consult a doctor. The question often is “who should you see?” For answers, Medical experts weigh-in on a recently published article from The U.S. News & World Report.  

Before you begin, start with understanding the role of your primary care physician and his/her expertise. Primary care physicians are typically internal medicine doctors, family medicine doctors or pediatricians. Internal medicine doctors, also called internists, handle common and complex illnesses in adults and older adults. Family doctors care for people of all ages from birth to end of life, and pediatricians specialize in the care of children. 

Experts say when you schedule a visit to your primary care doctor; they can help decide whether you need the expertise of a medical specialist. Dr. David Cutler, a board-certified family medicine physician at Providence Saint John’s Health Center in Santa Monica, California, explains upwards of 95% of cases can be taken care of with good primary care and the referral is the exception. 

The exceptions for the services of a medical specialist include when the patient’s chronic disease requires a special test such as a colonoscopy, stress test or an Upper GI Series test. If results show a need for treatment, the specialist may take over. Another reason a specialist is used is when the patient’s symptoms are eluding their primary care doctor; or the patient wishes for a second opinion.

A Tasty Addition in Your Diet Could Decrease Your Risk for Alzheimer’s

Recent research out of the UK reports there are things we can do that may influence our risk for Alzheimer’s disease. Although Alzheimer’s Research UK states there’s no sure-fire way to prevent dementia yet, because some of the things that shapes our risk like our age and our genes, can’t be changed. “But others, like our diet and the things we do to challenge our brains, can be.” They go on to say the latest evidence suggests up to 40% of all cases of dementia are linked to factors we may be able to influence.  

A tasty way to perhaps decrease our risk is to enhance our diet with the addition of several spices to our daily meals. The first of these spices is turmeric, which has lots of research behind it for offering antioxidant and anti-inflammatory properties. Turmeric contains an active, natural compound (polyphenol) called curcumin and is part of what gives turmeric its yellow hue. Mary-Eve Brown, an oncology clinical dietitian/nutritionist at Johns Hopkins Medicine, says some research shows a decrease in pain for patients with osteoarthritis when eating turmeric in recipes. Doctors suggest getting turmeric naturally in your diet rather than in supplements is healthier. 

The world’s most expensive spice, saffron, is also associated with health benefits including reducing depression symptoms and improving cognitive function; and gives food seasoned with it a golden hue color. The commonly used pepper is another spice on the list of beneficial foods as it may improve brain function and lower depression symptoms. Its value increases when used with turmeric as it “improves the bioavailability of nutrients in other foods, meaning your body absorbs more of the good stuff from what you eat.” Studies find another common spice, sage, may improve memory in low amounts.

This is where it helps to be creative, and remember gifts of your time often mean the most to an elderly person. However, there are some good gift ideas you may not have thought about and would be appreciated by your loved one. One idea for a mom or dad who loves coffee but doesn’t want to brew a whole pot is a single-serve coffee maker. Add in a few boxes of single-serve cups to use with the maker including hot chocolate or flavored coffees in a selection they would enjoy.

Mitigating Fall Risk in Older Adults

The Centers for Disease Control and Prevention (CDC) reports that each year, one out of every five Americans aged 65 years and older has a fall that results in an injury such as broken bones or a head injury.  However, medical professionals say while it is important for seniors to take precautions to prevent falls as injuries can be serious, they also remind us that regardless of how old we are, it is beneficial for our overall health to keep moving.  Experts explain that failing to engage in “mild exercise on a regular basis results in reduced muscle strength, decreased bone mass, loss of balance and coordination, and reduced flexibility.”

They go on to say that despite commonly held beliefs, falls are not a normal part of aging and there are things you can do to help keep you on your feet and mitigate the risk of a fall.  Their first piece of advice is to talk to your doctor about your risk and prevention.  Tell them if you have fallen and discuss your medications to see if side effects include dizziness or sleepiness that could contribute to falls.  

While falls are common in seniors 65 years of age and older, individuals with dementia are at an even greater risk.  Researchers say there are several reasons for this including gait and balance issues as well as problems concentrating and staying focused on the task at hand.  Other issues that may increase the risk of falls include poor eyesight and chronic diseases such as Parkinson’s, and arthritis or numbness in the feet.  If you often feel dizzy or lightheaded upon standing or suddenly sitting up, ask your doctor if you might have postural hypotension, and find out how you can alleviate the symptoms.    

Further tips to reduce your risk include having your eyes checked annually and updating your glasses when needed.  In addition, talk to your doctor about the best footwear for inside the house as well as outside.  Check your home for trip hazards (the CDC has a list of tips) and install grab bars in the bathroom.  Finally, ask your doctor about exercises to strengthen your leg muscles as well as balance-training exercises to improve balance.

Five Numbers that Can Impact Your Health

Medical experts say there are five numbers adults should know that can impact their health. Dr. Deepak L. Bhatt, editor-in-chief at the Harvard Heart Letter and part of the interventional cardiovascular programs at Harvard-affiliated Brigham and Women’s Hospital, explains, "For my patients, I typically look at their blood pressure, blood sugar, LDL cholesterol and triglycerides values, and their waist circumference."  

While there are specific recommendations, doctors also say the ideal values are good goals for most people but your doctor may suggest different numbers based on your age and other health conditions. Current recommended blood pressure numbers are less than 120/80 mm Hg. Systolic blood pressure is the first number and diastolic blood pressure the second number, and together they measure how hard your heart is working and the condition of your blood vessels. High blood pressure (HBP) can damage blood vessels and increase a person’s risk for a stroke or heart attack. 

Your LDL cholesterol numbers should be less than 100 mg/dL and your triglycerides less than 150 mg/dL. A cholesterol test is done through a blood draw after a person has fasted and if both your numbers are high, it increases your risk for a heart attack or stroke. Another important number is your blood sugar levels, which is how diabetes is typically diagnosed. Doctors say blood sugar levels should be less than 100 mg/dL.  

Finally, the last number you should pay attention to for better health is your waistline measurement. Experts say the circumference should be either lower than: less than half your height in inches; or for women less than 35 inches, and for men less than 40 inches. The problem with big numbers is what doctors call “abdominal or visceral obesity” typically an indication of fat surrounding the internal organs. Use a tape measure placed just above your navel (belly button) to get your number.

Is there Any Benefit From Taking Multivitamins?

As we age, it becomes more important than ever to protect our health but making decisions on how we do it can be confusing at best. One day it seems this diet is better than another and they have lots of initials such as DASH, MIND, TLC or KETO as well as Mediterranean, vegan, vegetarian and flexitarian diets. Adding to the challenge is whether we need a daily multivitamin. 

While it is a good idea to get advice from your healthcare provider, it doesn’t hurt to hear what some other experts say. For example, the Harvard School of Public Health says if you eat a healthy diet, a multivitamin may have little or no benefit. A healthy diet should include plenty of vegetables, fruits, whole grains, good protein sources and healthy fats. 

However the National Academy of Medicine cautions some Americans may not be getting enough of specific vitamins and minerals such as vitamin D and vitamin E from food sources alone. Vitamin deficiency can also occur in people with diseases like celiac, ulcerative colitis, or cystic fibrosis, which cause their digestive systems to not function properly. Others who may benefit from the addition of specific vitamins include the elderly or conditions such as pregnancy, alcoholism and taking certain medications including treating high blood pressure. 

Doctors say even those with diseases such as osteoporosis may only need to increase their intake of nutrients targeted toward maintaining bone health, or if that is not possible to take a supplement. If in doubt, ask your healthcare team if you should consult with a dietician. Remember to let your doctors know about any supplements you are taking in case of drug interactions with prescription medications.

If you are moving from your own home into a senior living community, most likely you will want to downsize. While it can sound overwhelming to go through a whole household of stuff and whittle it down to fit into a modest sized apartment, a few tips will help. 

Without question it is a lot of work and can be stressful and emotional but the Internet offers useful information from organizing experts. Most say the trick is to prepare in advance by making a plan and allowing plenty of time. Remember, if you are interested, there are companies who offer these services and will do everything from helping with the downsizing process to packing up, and moving your things to your new home. 

Now you are ready for the sorting out process deciding what you will bring and what to give to family, donate, or throw away. This is the hardest task as your house is full of memories but again here is where a system works. Experts say sort by rooms and simplify the task by making a “yes” pile and a “no” pile. They warn not to make a “maybe” pile as it can end up growing in size and complicate your plan. Again, remember the goal is to simplify your life and not overcrowd the new place with useless items.   

Things to consider are the size of your new place and its layout (get measurements). You will want furniture scaled to fit as well as room for hobbies or musical instruments that are important to you. Also consider the amenities your new place offers such as a dining service. If most of your meals will be prepared by your new community, you will just need the basics in your kitchen. If the new place has a fitness center, there is no need to bring your own equipment.
