Questions Regarding What Medicare Does and Doesn’t Cover?

Many seniors new to Medicare have questions about the scope of what is covered under the program. Experts explain Medicare covers the majority of seniors’ health care needs including hospital care, doctor visits, lab tests and prescription drugs. However there are some things not covered and are included in the following.

While Medicare does cover treatment and doctor visits regarding medical problems in the ears; many seniors are surprised to learn it does not cover hearing aids or routine ear exams. Hearing aids are expenses and one solution to defray the cost is to purchase Medicare gap insurance or a membership in a discount plan that helps cover the cost of hearing devices. In 2017 a bill was passed that allows hearing aids to be sold over-the-counter and without a prescription for people with mild to moderate hearing loss; greatly reducing the cost.

Eye exams, glasses and contact lenses are another one of the things not covered under traditional Medicare. Medicare does cover things like cataract surgery and other medical conditions of the eye. If you are concerned about the cost of glasses and exams, vision insurance is available. The same goes for dental care, and there are additional plans out there to help cover the cost of dental work. 

If you travel out of the country, you need to know Medicare will not cover any of your medical care. Frequent travelers recommend purchasing a Medigap policy that covers certain overseas medical costs; or check into travel insurance policies offering basic health care. Some seniors opt to pay upfront for emergency medical evacuation insurance. 

Other types of services not covered by Medicare are podiatry, massage therapy and chiropractic care. While short-term rehab services are covered, Medicare does not cover long-term care in retirement homes.

What Information is needed if Mom or Dad is hospitalized?

While many hospital stays are planned, older adults are more likely to require an unexpected trip to the hospital. When you have the luxury of time, it is easy to get prepared with the necessary information a parent needs when admitted to the hospital. It can be a different story if the trip is sudden. 

Medical experts say to ensure a loved one gets the proper care, it is important to see to it that the hospital has a complete medical history so this is one of the things to start gathering. For starters, the hospital will want a “next of kin” contact for the patient so decide beforehand who that will be.  

Now go through the list of medications your parents are taking and make sure they are all together in one location. In the event of a medical emergency, you will be able to bring all of their medications with you to ensure the hospital knows the most up-to-date medicines and dosages the person is taking. 

Another thing to gather information on regards your parent’s pre-existing conditions and any recent surgeries or procedures they may have had. Provide the name of their primary care provider; and be sure to let the hospital personnel know if your parent has dementia or any physical limitations. 

If the hospital doesn’t already have a copy of your loved one’s advanced directive, also known as living will, bring one with you. An advanced directive is a legal document in which a person specifies what actions should be taken in end-of-life situations when they cannot speak for themselves. Mom or dad may also have a Do-Not-Resuscitate Order, which is another kind of advance directive. It covers not receiving cardiopulmonary resuscitation (CPR) if the person’s heart stops or if they stop breathing. 

Other information to give to the hospital regards allergies and dietary restrictions. Additionally make sure your loved one has their cell phone with them as many hospital rooms do not have phones.

Early Signs of Parkinson’s Disease

The Parkinson’s Foundation reports there are almost one million people in the United States living with Parkinson’s disease; and that number is expected to increase to 1.2 million by 2030. However they say the actual number of people with PD is likely to be higher as many cases go undiagnosed.

The Mayo Clinic explains Parkinson’s disease is a progressive disorder that affects the nervous system and the parts of the body controlled by the nerves. Currently there is no cure but medications can improve symptoms, and in some cases, surgery may also improve symptoms.  

Parkinson’s symptoms typically start slowly, and one of the first things a person may notice is a slight tremor in one hand. Called resting tremors, they occur when the person’s hands are at rest, and the first tremors may only be the shaking of one finger. It is important to note that not all tremors are caused by Parkinson’s and you need to consult with your healthcare provider to see if it is Parkinson’s or something else causing the condition. 

Stiffness or rigidity, and slowed movement known as bradykinesia are other common symptoms of the disease. Rigidity occurs in the shoulders, trunk, limbs and neck. Bradykinesia includes slowness in everyday movement and tasks such as walking, speech and eating. Another common sign of Parkinson’s is a change in the person’s handwriting. Small handwriting called micrographia occurs because of changes in the parts of the brain that control motor skills. As the disease progresses, people report problems with sleeping, mood swings and changes in cognitive skills as well as problems with vision. 

The risk of Parkinson’s increases with age and most cases are diagnosed after the age of 60. Men are more likely to come down with the disease, and there is a small hereditary link. Moreover, ongoing exposure to pesticides and herbicides may increase your risk.

New Advances in the Treatment of Alzheimer’s

While treatments to help in the fight against Alzheimer’s cannot come quickly enough for families of those with the disease, researchers say there are lots of drugs in the pipeline designed to not only treat symptoms but perhaps eradicate some of the damage. The Mayo Clinic explains current treatments only temporarily improve problems with memory and reasoning and are far from a cure. However there is cautious hope that researchers may discover treatments that will stop or delay the progress of the disease. 

Today’s advancements are geared toward working on a better understanding of the disease and how it changes the brain; and then trying to design potential Alzheimer’s treatments targeted at the process. Scientists believe future treatments may look somewhat like treatments for AIDS/HIV and some cancers, which use a multi-medication approach.

One current research project includes targeting the protein beta-amyloid, known as plaques, in the brain. Scientists are trying to use medicines called monoclonal antibodies to prevent the beta-amyloid from forming plaques. The medications could also be used to break up already formed clumps of plaques and help clear them from the brain.

Another possible treatment for Alzheimer’s is a medication used to treat cancer. In a study using mice, saracatinib turned off the production of protein beta-amyloid; and allowed synapses to start working again. The synapses are tiny spaces between brain cells that help the cells to communicate. The animals in the study showed a reversal of some memory loss, and researchers have started clinical trials in humans.  

From studies, researchers know a protein called tau twists into tiny tangles in the brains of those with Alzheimer’s. Now they are looking at ways to prevent this from happening by using tau aggregation inhibitors and tau vaccines in clinical trials.     

Additional studies are looking at the connection between head and heart health. Strategies include studying medications that help lower blood pressure and other drugs used to treat heart disease; and whether they can help Alzheimer’s patients. More research is also studying the connection between a healthy lifestyle and diet in lowering a person’s risk, or delaying the onset, of dementia.

Vitamin D, How Much Do You Need and How to Get It

There has been much in the news lately about Vitamin D including how much we need each day and the fact that far too many people may be deficient. Medical professionals say there are not many foods high in Vitamin D so relying only on what we eat can be challenging. It is estimated close to half of all Americans are deficient in D; but without a blood test, it is difficult to know for sure. 

Vitamin D is good for us as it contributes to bone health by aiding our bodies with absorbing calcium and phosphorus. It additionally helps nerves carry messages between the brain and other parts of the body as well as fights inflammation.  

A food high in D is rainbow trout. Dieticians say one 3-ounce serving contains 645 IUs, which is more than the daily recommended serving for adults. Other foods high in D include salmon, which is high in vitamin D and omega-3 fatty acids, egg yolks and canned tuna. Mushrooms grown under UV light also have a good amount of vitamin D. Another source is food fortified with D such as many types of milk, yogurt, cereals and orange juice. 

If you are concerned about sun exposure due to the risk of skin cancer and don’t want to get vitamin D that way, experts say it only takes about 10 to 30 minutes of daily direct sunlight exposure to ensure vitamin production. The National Institutes of Health recommends a daily allowance of 600 international units IU (15 micrograms) of vitamin D for most adults. For seniors 70 years and older, 800 IU (20 micrograms) is the daily recommendation. Ask your doctor for a blood test to determine if and how much vitamin D you need.

Mobility Issues as We Age

Luckily for most of us, when we are younger we take our mobility for granted. We didn’t have to think about whether or not we will be “able” to physically do a routine task. However as we grow older, our bodies can encounter physical limitations that threaten our independence. Experts say protecting our mobility requires attention and the “use it or lose it” mantra is a fitting one. Lifestyle choices and exercise as well as diet and preventive care can help keep us stronger and steadier with fewer aches and more stamina. 

The Harvard Health Publishing by Harvard Medical School defines mobility as the ability to move purposefully as you go through your day, and is the foundation for living a healthy and independent life. It requires a certain amount of physical stamina, strength, balance, coordination, and range of motion. Mobility allows seniors to do everyday things like get out of bed each morning, climb a set of stairs and bring in groceries from the car. Poor mobility causes problems such as being unable to get up out of a chair on your own or get up from the toilet without a helping hand. Good mobility helps keep seniors safe by lessening their risk for falls and subsequent injuries; and allows the person to live on their own.                                                                

The US Census Bureau reports mobility issues are the most frequent cause of disability for people over 65 and impacts their ability to safely climb stairs and walk. While some mobility problems can’t be helped, experts say generally to maintain mobility, seniors should do exercises targeted to increase strength, balance, stamina and coordination. 

Ask your doctor, physical therapist or trainer on ways to preserve and improve your freedom of movement. Moreover if you are unsteady on your feet, check them about using a mobility aid such as a cane or walker to improve steadiness.

Studies Show Brain Games can Improve Cognitive Skills

Over the last couple of decades, researchers have found brain-training games may help improve attention levels, logic skills, and memory as well as enhance cognitive function in healthy subjects. Shaheen Lakhan, MD. Ph.D. FAAN, a physician-scientist and clinical development specialist and founding neurology department chair and curricular dean at California University of Science and Medicine writes the following: “The brain is just like a muscle – it thrives on exercise! As a neurologist, I’m thrilled by the incredible potential of brain games to help people flex their mental muscles, activating underused brain circuits to sharpen cognition and skills like focus, speed, and memory.”

Experts say effective brain games include Sudoku and crossword puzzles. With crossword puzzles, cognitive skills used include recollecting past verbal knowledge and memories. Sudoku relies on short-term memory, which is often one the first sign of Alzheimer’s, and requires the player to look ahead to plan out subsequent moves.

Android and iOS offer a number of apps for brain games that are either free, include in-app purchases or require a subscription to open up more games. One game is Elevate that centers on reading, writing, speaking, and math; and allows you to customize training on areas you wish to focus on. Another game, Luminosity, was involved in a recent study where people who played the games for “15 minutes a day at least seven days a week for three weeks experienced improved attention and motor speed.”

Women and their Bone Health

Endocrinologist Kendall Moseley, MD, medical director of the Johns Hopkins Metabolic Bone & Osteoporosis Center, says most women do not know anything about their bone health. A recent survey from the National Osteoporosis Foundation backs up that sentiment finding around 94 percent of women do not worry about their skeletons. However, they add they should, as one in five women under 30 already have osteopenia, which is a step above full out osteoporosis. The good news is there are things you can do to protect your frame and avoid the things that are not good for bone health.  

Experts say our bones constantly break down and rebuild every 10 years resulting in a new skeleton. The problem lies in the fact that after the age of 30, our bodies start to lose more bone than they make. Women’s estrogen levels also begin to drop around 30, which is another protector of women’s bone health. 

An unbalanced diet or severely restricting calories in an effort to stay thin or lose weight can be another cause of low bone density; and too much caffeine, salt and alcohol all interfere with retaining calcium, our primary bone builder. Even some medications deplete calcium and include certain antidepressants and contraceptives so be sure to ask your doctor about side effects of medications you take. It is important for women to include dairy products or foods like beans and nuts to increase their calcium intake. Protein from plants and lean meats are also good ways to get calcium.

Along with a good diet, exercise is an important element of keeping bones strong. While some exercises are better than others, scientists recommend weight bearing exercises like walking and running as good ways to build strong bones. Strength training exercises may be the ultimate bone builder and include things like pushups and squats or kickboxing.

Screenings Guidelines for Women

Medical professionals stress the importance of women getting the proper screening to protect their health and discover problems early when they may be more treatable. Screenings are designed for otherwise healthy individuals and recommendations change as we age. The publication US News & World Report, states, “Your doctor can help you tailor the recommendations based on your goals of care, personal and family health history, age and life expectancy.”

One of the most recommended screenings for women over the age of 40 is to have an annual mammogram. Doctors say mammograms are key to the prevention as well as early detection of breast cancer when it is most easily treated. Talk to your doctor about testing options if you are under the age of 40 and have a mother or sister who contracted breast cancer at a young age. 

It is recommended women 50 years and older have a colonoscopy every ten years. As colon cancer is more common in African-Americans and occurs at an earlier age, it is advised to start testing at age 45. Younger women/teens with a family history of adenomatous polyposis – a genetic condition that starts in someone as young as in the teen years may need colonoscopies at a much earlier age. Also a woman with a history of Crohn’s disease or inflammatory bowel disease will likely be advised to have a colonoscopy at a younger age than 50.    

Diabetes is another disease a woman 45 years and older should be screened for every three years. Exceptions for screening earlier are a family history of diabetes; obesity and high blood pressure readings or you take medications for HBP.

A Lipid profile assessment testing a woman’s cholesterol and triglyceride levels as well as the risk for heart disease should be done every five years starting at age 45. If lifestyle changes do not reduce your numbers, you may need medications. As with all screening, discuss with your doctor how often you need to be screened and what steps to take to improve your health.

The Lancet journal reports the most common form of arthritis is osteoarthritis (OA); and it is estimated to affect 500 million people worldwide (7% of the global population). In the US, over 32.5 million have OA, or 1 in 5 adults. Of Americans with OA, 70% are over the age of 55; and it is the leading cause of disabilities in seniors.  

Females are more likely to have OA with many cases occurring after menopause. Research suggests the resulting physiological changes in a woman’s body accounts for the elevated number of cases. Obesity is another risk factor, and may be due to the increased stress on weight-bearing joints. Others are more at risk for OA due to their occupation such as agricultural work, cleaning, construction and carpentry.

OA develops as the cartilage covering the end of a joint bone begins to wear away; and the joint rubs against a rough surface rather than smooth cartilage. With repeated use of a joint and the changes in the tissue over time, the condition is more common in older adults. Pain and stiffness associated with OA can impact a person’s day-to-day life and lead to depression and anxiety as well as make it harder for them to control their weight.  

While exercise has been shown to reduce pain in people with OA, it also comes with challenges. Many fear making the pain worse, and instability can cause a person to feel like they may fall or contribute to a lack of range of motion and mobility. 

However exercise can be very beneficial to people with OA. Julie Lombardo, a physical therapist and CEO of Capitol Physical Therapy in Madison, Wisconsin, says, “It’s easy for people who encounter hurdles to give up on exercising when that is the last thing they should do.” Some exercises are better than others for people with OA; and consult with your healthcare provider if you are new to exercise, have high blood pressure or heart disease, or are obese.

A Tasty Addition in Your Diet Could Decrease Your Risk for Alzheimer’s

Recent research out of the UK reports there are things we can do that may influence our risk for Alzheimer’s disease. Although Alzheimer’s Research UK states there’s no sure-fire way to prevent dementia yet, because some of the things that shapes our risk like our age and our genes, can’t be changed. “But others, like our diet and the things we do to challenge our brains, can be.” They go on to say the latest evidence suggests up to 40% of all cases of dementia are linked to factors we may be able to influence.  

A tasty way to perhaps decrease our risk is to enhance our diet with the addition of several spices to our daily meals. The first of these spices is turmeric, which has lots of research behind it for offering antioxidant and anti-inflammatory properties. Turmeric contains an active, natural compound (polyphenol) called curcumin and is part of what gives turmeric its yellow hue. Mary-Eve Brown, an oncology clinical dietitian/nutritionist at Johns Hopkins Medicine, says some research shows a decrease in pain for patients with osteoarthritis when eating turmeric in recipes. Doctors suggest getting turmeric naturally in your diet rather than in supplements is healthier. 

The world’s most expensive spice, saffron, is also associated with health benefits including reducing depression symptoms and improving cognitive function; and gives food seasoned with it a golden hue color. The commonly used pepper is another spice on the list of beneficial foods as it may improve brain function and lower depression symptoms. Its value increases when used with turmeric as it “improves the bioavailability of nutrients in other foods, meaning your body absorbs more of the good stuff from what you eat.” Studies find another common spice, sage, may improve memory in low amounts.

Online Games & Activities can Help Keep Your Brain Sharp

As we age, many of us find ourselves more absentminded and forgetful than we were in our younger years. Seeing someone unexpectedly from our past can result in recognizing the face but being unable to bring a name to mind. Experts say some lapses in memory are common as we age but encourage seniors to do cognitive activities to help stay sharp. 

Brain games exercise your memory, attention, brain speed, people skills, intelligence and navigation just like a trip to the gym gives you a cardio workout and exercises your abs and quads. One good resource for such games is the AARP online site. It offers a variety of fun and entertaining games and puzzles such as daily crosswords, word games like scrabble, Sudoku and trivia as well as the opportunity to join a bridge, chess or backgammon game. 

The New York Times offers the daily Wordl puzzle that gives your brain a  quick workout along with several other word puzzles for Times’ subscription holders. Other programs offering brain games targeted to seniors that require a monthly or yearly subscription. Protect Your Medicare Card like a Credit Card

The National Health Care Anti-Fraud Association estimates last year’s annual Medicare insurance fraud was around $100 billion, and most likely much higher. While one in five beneficiaries say they have encountered potential Medicare fraud, there are precautions you can take to reduce or eliminate fraudulent claims starting with protecting your Medicare and Social Security cards and numbers like you do your credit cards. 

Experts say don’t respond to a call, text or email asking you to provide your Medicare Number. They caution you to only show your Medicare card or give out your number to your doctor or people you know who should have it. To determine if you are a victim of fraud, examine your Medicare Summary Notices (MSNs) or claims statements carefully for any discrepancies or additions of services or charges you did not receive. Another common scam is to try to get Medicare beneficiaries to sign up for drug plans that are not approved by Medicare. 

If your Medicare card is stolen or you suspect fraud, report it as soon as possible to 1-800-MEDICARE (1-800-633-4227). It is important to know Medicare will never call to sell you anything or visit you at home. They will only call or contact you in limited situations to ask for personal information. One such person may be the agent who helped you sign up for a particular Medicare program or drug plan. You also may be contacted by Medicare if you left your number to report fraud or inconsistencies with your MSNs.

Hearing Loss is Associated With Cognitive Impairment

JAMA Neurology added to the growing body of evidence showing a link between hearing loss and cognitive impairment. The study found treating hearing loss with hearing aids may reduce the risk of developing dementia by up to 19%. In fact, researchers say hearing loss may be the biggest treatable factor for dementia. An article in AARP explains what may cause the link between hearing loss and dementia. 

The first theory regards socialization. Scientists say once a person starts having more difficulty being in a group of people and following the conversation, they may avoid such social gatherings. This in turn can lead to isolation, which is problematic as less social engagement as well as less cognitively stimulating activities and depression are risk factors for dementia. 

Secondly, coping with hearing loss means the person is constantly working to sort out sounds and make sense of what is being said. The brain now may have “fewer resources (brain power) to help preserve thinking and memory abilities.” Scientists also believe hearing loss causes the brain to be less stimulated with sound information, which is linked with parts of the brain shrinking and atrophying faster with chronic deprivation.  

While experts agree we still need more studies regarding the link between hearing loss and a higher risk for dementia, they do know that addressing a hearing issue with hearing aids is beneficial for the person’s overall quality of life. Moreover, there are no negative medical issues associated with the devices.

Reducing the Risk of a Fall

The Center for Disease Control & Prevention (CDC) reports millions of older adults fall each year—resulting in more than 36,000 deaths. Of those falls, 3 million seniors are treated in emergency departments for fall injuries; and one out of every five falls causes an injury, such as broken bones or a head injury. With these alarming statistics, experts say it is important for seniors to know their risk for a fall including whether they are taking medications associated with an increased risk. 

The first step is to talk to your doctor to see if you are taking one of these medications, as medications are a common reason for falls. Furthermore, experts say adjusting or making a change in medication is the easiest thing to do to help prevent a fall. If it is determined you are taking a medication known to cause light-headedness, dizziness, sudden drop in blood pressure, or chronically low blood pressure or hypoglycemia (low blood sugar), you and your healthcare team can regularly re-evaluate your need and dosage. 

Another way to mitigate your risk for a fall is to do exercises designed to improve leg strength and balance. In addition, be sure to get an annual eye exam and update your glasses as needed. Lastly check your home for trip hazards (keep clutter out of hallways and stairways) and install grab bars and better lighting where needed.

A Positive View on Aging May Help You Live Longer

Social scientists say “aging gracefully” should not be just an expression but rather something people should take to heart as they enter their senior years. Maintaining a positive attitude as we age can help us better weather the many ups and downs we encounter along the journey. 

A comprehensive study from the Yale School of Epidemiology and Public Health followed 660 seniors aged 50 years and older and found those who held a more positive view on growing older lived on average 7.5 years longer than those with a negative view. Researchers added negative views on old age may even make someone more susceptible to developing the brain changes associated with Alzheimer’s disease. However, they report if a person shifts to a more positive way of thinking, the adverse outcome may be mitigated.   

Dr. Manfred Diehl, professor of human development and family studies at Colorado State University, advises people on how to develop a more positive approach to aging. His first recommendation is to be physically active at least 30 minutes each day. Then he says to keep your brain active and challenge it by continuing to learn new things. Other tips include: get enough sleep each night, stop smoking, avoid excessive alcohol, maintain a healthy weight and follow a healthy diet. Dr. Diehl says a healthy social life is important and to nurture a healthy relationship with your spouse, family and friends.   

Another positive step is to get regular medical check-ups and screenings. Additionally, protect your mental health by taking the time to relax, unwind and to have fun.
