Aging and Muscle Loss

The term sarcopenia refers to age-related muscle loss and is a natural part of growing older. However, it also may occur at a faster pace after prolonged periods of inactivity from injury, illness or the pain of osteoarthritis. Sarcopenia causes muscle atrophy and brings on problems such as poor balance and greater weakness, which in turn, can result in more inactivity and increased risks for falls.   

Seniors with muscle atrophy may have difficulty reaching their arms upward, opening jars, typing on a keyboard or even tying shoes.  The Journal of Rehabilitation Medicine reports on a study that found “older men who did eight weeks of strength training lost about 25% of their muscle gains after they stopped training for two weeks.”

Jodi Klein, a physical therapist with Harvard’s Brigham and Women’s Hospital, suggests a few things you can do if you know you have a surgical procedure ahead of you that could cause muscle atrophy. She says if you are currently active, keep it up; if not, use this time to get started.  Klein explains, “The better shape you are in going into surgery, the better shape you will be coming out, and the less chance you have of enduring muscle loss.” 

Older adults who want to regain muscle strength should talk to their health care provider about the best methods. Experts recommend an exercise routine that builds both upper and lower body strength. This could include weight training with dumbbells and resistance bands in conjunction with walking, swimming, and biking. If it would be helpful, think about turning to an expert to aid you in regaining strength.

If You Are Experiencing Memory Problems

If you or a loved one is experiencing troubling memory problems, it is important to see your healthcare provider. While there is no cure for Alzheimer’s or other dementia-related diseases, there are steps you can take to mitigate or delay the onset of the disease. 

The Alzheimer’s Association explains, “Physicians and other medical professionals are a central part of the care team of a person living with dementia. Along with other members — which may include family, friends or others — they work to provide support throughout the course of the disease.” Therefore, one of the best ways to ensure optimal care is to honestly communicate with your physician.  

To establish a good relationship with the doctor, a person or family member should take some time to prepare for the visit. Helpful tips from the Alzheimer’s Association include:

  • Document any changes in your health, including your mood, memory and behaviors.

  • Make a note of any changes you’ve noticed since your last appointment.

  • Create a list of current prescriptions and over-the-counter medications.

  • Write down any vitamins or supplements you’ve added to your medication regimen.

  • Make a list of questions you want to ask.

Writing down questions you have beforehand makes sure you don’t forget them, and don’t be reluctant to ask questions if you don’t understand something. The Alzheimer’s Association National Early-Stage Advisory Group, made up of men and women in the early stages of Alzheimer’s or other forms of dementia, put together a list of questions to help others make the most out of a doctor’s visit. Questions range from asking about the type of tests used to make a diagnosis, what to expect from the disease, and what type of treatments may be used. When seeing a specialist, ask how familiar they are with caring for patients with Alzheimer’s disease.

Is there Any Benefit From Taking Multivitamins?

As we age, it becomes more important than ever to protect our health but making decisions on how we do it can be confusing at best. One day it seems this diet is better than another and they have lots of initials such as DASH, MIND, TLC or KETO as well as Mediterranean, vegan, vegetarian and flexitarian diets. Adding to the challenge is whether we need a daily multivitamin. 

While it is a good idea to get advice from your healthcare provider, it doesn’t hurt to hear what some other experts say. For example, the Harvard School of Public Health says if you eat a healthy diet, a multivitamin may have little or no benefit. A healthy diet should include plenty of vegetables, fruits, whole grains, good protein sources and healthy fats. 

However the National Academy of Medicine cautions some Americans may not be getting enough of specific vitamins and minerals such as vitamin D and vitamin E from food sources alone. Vitamin deficiency can also occur in people with diseases like celiac, ulcerative colitis, or cystic fibrosis, which cause their digestive systems to not function properly. Others who may benefit from the addition of specific vitamins include the elderly or conditions such as pregnancy, alcoholism and taking certain medications including treating high blood pressure. 

Doctors say even those with diseases such as osteoporosis may only need to increase their intake of nutrients targeted toward maintaining bone health, or if that is not possible to take a supplement. If in doubt, ask your healthcare team if you should consult with a dietician. Remember to let your doctors know about any supplements you are taking in case of drug interactions with prescription medications.

Many ideas for someone with Alzheimer’s or dementia include personalized gifts.  One such gift is a talking photo book that allows you to record your own personal commentary to go along with each photograph.  This can be a fun book to sit and enjoy with your family member as it helps to trigger memories and conversation.  Many companies offer such books, and prices typically vary with the number of photos and recording time available.  

Buying an interactive book featuring vivid photographs and large print on a subject your mom or dad loves such as animals or flowers can also promote conversation and provide a way to spend relaxing time together.  To ease stress or agitation, you may want to consider getting them a fiddle muff.  A fiddle muff can provide comfort and warmth as well as activity to promote increased hand flexibility and brain stimulation.

A Fall Can Result in Serious Injuries

A fall can result in serious injuries especially for the elderly, who make up most cases, and can take a long time to heal.  Many even lead to permanent mobility issues that limit the person’s freedom.  The CDC reports that each year 3 million older adults are treated in the emergency room for a fall injury and over 800,000 patients are hospitalized due to their injuries. 

Experts explain there are several reasons why the elderly are more vulnerable to falling and include medical conditions such as Parkinson’s and heart disease, arthritis, osteoporosis, hypoglycemia, diabetes, and blood pressure fluctuations.  In addition, physical disabilities that affect strength and balance, poor eyesight, inner ear problems, and dehydration can also put a person more at risk for a fall.  Common situations that can result in a fall include wet or recently polished floors, poorly lit areas, rugs not properly secured, stairs, and uneven sidewalks. 

There are some changes you can make to fall-proof your home such replacing light bulbs in dimly lit hallways or stairways with higher wattage bulbs.  Clean up clutter and make sure each room in your home has a clear, clean path to walk through and install grab bars and handrails where needed.  Use a non-slip mat in the bathtub or shower and do not wear socks or loose, floppy slippers but rather a good walking shoe with support and cushioning.   

If a loved one is unsteady on their feet, talk to their doctor about using a walker or cane.  Also, ask if any of their medications can cause dizziness or light-headedness, and if so, is there an alternative.  Doctors say if not, it is even more important to make the appropriate lifestyle changes.

Making the Move to a Senior Community

If you are moving from your own home into a senior living community, most likely you will want to downsize. While it can sound overwhelming to go through a whole household of stuff and whittle it down to fit into a modest sized apartment, a few tips will help. 

Without question it is a lot of work and can be stressful and emotional but the Internet offers useful information from organizing experts. Most say the trick is to prepare in advance by making a plan and allowing plenty of time. Remember, if you are interested, there are companies who offer these services and will do everything from helping with the downsizing process to packing up, and moving your things to your new home. 

Now you are ready for the sorting out process deciding what you will bring and what to give to family, donate, or throw away. This is the hardest task as your house is full of memories but again here is where a system works. Experts say sort by rooms and simplify the task by making a “yes” pile and a “no” pile. They warn not to make a “maybe” pile as it can end up growing in size and complicate your plan. Again, remember the goal is to simplify your life and not overcrowd the new place with useless items.   

Things to consider are the size of your new place and its layout (get measurements). You will want furniture scaled to fit as well as room for hobbies or musical instruments that are important to you. Also consider the amenities your new place offers such as a dining service. If most of your meals will be prepared by your new community, you will just need the basics in your kitchen. If the new place has a fitness center, there is no need to bring your own equipment.

Brighten the Day of Your Favorite Senior

Do you have an elderly relative living alone or in a senior community who lives too far to visit?  How about sending something special to show your thinking of them even if it’s not their birthday or a holiday.  One idea is to send them a gift basket filled with goodies to eat and drink, gifts to entertain, and items to make their lives a little easier or more pleasant.  

For your coffee drinking parent or grandparent, a bag of gourmet coffee and a nice new mug is one way to start your basket, or add a collection of teas that include decaffeinated and herbal options.  Do they love chocolate or prefer salty snacks?  Adjust your selection to include high-end chocolates and other delightful treats or a nice selection of crackers and cheeses.  For a book lover, add a novel you think they would enjoy or a coffee table book on famous gardens, architecture, maps, photography or wherever matches their interest.   

Now pack the basket with items such as jigsaw puzzles, crosswords, word-search books, a new coloring book and fresh colored pencils or markers.  If they have low vision, include large print playing cards and books.  If your loved one is feeling anxious or having a hard time sleeping, give them a weighted blanket.  While they come in many sizes, it is suggested you choose one that is 10% percent of their weight.   For example, if they weigh 150lbs, buy a 15lb blanket, and if they do not have a blanket that exactly matches 10% of their weight, you can go up a couple of pounds.

Exercise Helps Seniors Maintain Independence

While we know exercise is good for us, certain ailments can make it more challenging for some seniors. However, you may want to be mindful of advice from the Harvard Medical School that says, “Regular exercise promotes an older adult’s ability to walk, bathe, cook, eat, dress, and use the restroom. If self-reliance is a priority, exercise is one of the best ways to maintain independence for older adults.” In fact, study after study has found exercise offers older adults a longer, healthier, and happy life; but before you begin, please check with your doctor on the best ones for your level of fitness.   

Low impact exercises such as water aerobics and chair yoga are especially good for seniors as they offer lots of benefits and are easy on the body. With water aerobics, the natural buoyancy of the water puts less stress on the joints, and it can improve balance, flexibility and strength. Chair yoga puts less stress on the body than regular yoga and can easily being adjusted to the level of fitness for each person. It too offers many of the same benefits as water aerobics. 

An easy to start exercise that doesn’t require special equipment or skills is walking. While it can be done almost anywhere, studies have found walking on a nature trail offers physical exercise as well as a calming influence on the spirit. Another good exercise for seniors is Tai chi. According to the Harvard Medical School, it is shown to “strengthen the heart, improve balance and flexibility, slow the onset of dementia, reduces stress, lowers blood pressure, and relieves the pain of rheumatoid arthritis and fibromyalgia and more.” Other exercises for older adults include using resistance bands or doing Pilates to develop core strength and improve balance and flexibility.

What Are Cataracts?

Cataracts are cloudy areas that form in the lens, which is normally transparent, and are the main cause of vision loss in people over 40 years of age. It is estimated more than 40 percent of Americans aged 75 years or older have some degree of lens clouding; and over half of people 80 or older have their vision significantly impaired by cataracts or have had cataract surgery. Surgery is generally recommended once vision is impaired to a degree that people have trouble reading, driving and recognizing faces. Today the surgery is a quick, safe procedure, replacing the cloudy lens with a clear plastic one. 

Doctors say there are things people can do to reduce their risk of developing cataracts with quitting smoking at the top of that list. Medical News Today reports on studies linking a 3-fold increase of smoking to cataracts as well as raising the risk of several other eye conditions. A healthy diet of fresh fruits, vegetables, whole grains, good quality fats (avocados, olive oil, and omega oils) and lean animal-sourced or plant-sourced proteins additionally reduces the risk of cataracts. 

Maintaining a healthy weight is important as obesity increases your risk of developing type 2 diabetes, which is a risk factor for cataracts. Other ways to reduce your risk include wearing UV blocking sunglasses when in the sunlight and getting at least 7 hours of sleep each night. The risk for cataracts also increases with age, family history, and a history of eye inflammation or injury. 

If you are not ready for surgery yet, doctors say stronger glasses and brighter lights may help. Keep up with regular eye exams and renew your eyeglass prescription as needed. Wear sunglasses in the sun and avoid night driving. As your cataracts continue to develop and eyesight worsens, at some point you will most likely need surgery.

Is it a Good Idea to Purchase a Hearing Aid Over the Counter?

Did you know the FDA has made changes to regulations regarding hearing aids, which now makes it possible to buy an FDA approved device over the counter (OTC)? While many of us have heard about the high cost of prescription hearing aids and that Medicare typically does not cover them, you may want to know whether the experts think an OTC purchase is a good idea. 

The Harvard Medical School went to Dr. James Naples, assistant professor of otolaryngology/head and neck surgery at Harvard-affiliated Beth Israel Deaconess Medical Center, to see if it is a viable option and to explain potential pros and cons. He explains that despite there being several types of hearing aids, they all amplify sound and use a combination of signaling processing and directional microphones to filter out background noise. However, he warns against confusing personal sound amplification products (PSAPs) with hearing aids as they only amplify sound.

To determine your level of hearing loss, experts recommend you start by getting a hearing test from an audiologist like you would get an exam from an optometrist when buying eyeglasses. This is especially necessary for severe hearing loss, as OTC devices are not appropriate for all types of hearing loss. For mild to moderate hearing loss, it may be possible to get good results by purchasing an OTC device without a hearing exam and fitting by an audiologist. 

The next question routinely asked is if you will save money by purchasing an OTC hearing aid and the answer is most-likely yes as right now the average hearing aid runs anywhere between $2,000 to $4,000 per ear, and age-related hearing loss may be in one or both ears. OTC also helps reduce the price by eliminating the added expense of the hearing test along with the actual device being substantially lower. Doctors say the new regulations should increase competition and some estimates indicate prices could drop by as much as $1,600. 

Dr. Naples cautions that just as drugstore readers are a great option for many, some individuals have more complicated issues that require more precise solutions. Hearing aids are the same. The doctor states the safest bet is to get a hearing test to confirm an OTC device is right for you.

Yoga Benefits Both Your Body and Brain

While practitioners have known for centuries the calmness and peace yoga brings into their lives, science has finally caught up with real evidence of its benefits to our brains. Studies show yoga elevates levels of the brain chemical gamma-aminobutyric acid (GAGA), which helps regulate nerve activity and is associated with better mood and decreased anxiety. 

The Harvard Medical School journal writes, “Yoga strengthens parts of the brain that play a key role in memory, attention, awareness, thought, and language. Think of it as weightlifting for the brain.” Their evidence comes from several studies that used MRI and other brain imaging technology. Researchers found participants who practiced yoga regularly had a thicker cerebral cortex, the part of the brain responsible for learning and memory, than participants who did not practice yoga. 

This could be significant because as people age, this is one of the areas in the brain that typically shrinks. As the older yoga practitioners had less shrinkage than non-practitioners, researchers say it suggests yoga can “counteract age-related declines in memory and other cognitive skills.”  

Other benefits of yoga include helping individuals lose weight, sleep better, lower blood pressure and lower blood sugar levels to improve diabetes. It also helps people gain flexibility and muscle strength around arthritic joints and reduces the pain associated with the condition.

A meta-analysis of 39 studies on aging and resistance-type exercises such as lifting weights or using exercise bands, found it is possible to build muscle mass no matter what age. In the studies, “1,300 adults over the age of 50 who did some form of this kind of training, muscle mass increased by an average of nearly 2.5 pounds in just five months.”

Being positive is a trait that helps people deal more effectively with stress and worry and is linked to longevity. Researchers also find it is better for us to be in charge of our own medical care. One way to do this is to stay up-to-date on medical research and advice regarding care options as well as asking pertinent questions of our healthcare providers.
